II. Reason for referral


C.M. has attended an assessment to discuss both her personal and academic issues as well as
her recent experiences over the past few months. She experiences anxiety in addition to stress, which
negatively impacts her daily functioning including her relationship with others and her education. She
has also noticed her mood fluctuating lately.
III. Behavioral Observation
During the assessment, C.M. presented herself neatly groomed and appropriately dressed. She
appeared calm and relaxed while seated comfortably. Her speech was clear and normal when
answering questions about how she started her day and how she feels. She demonstrated good
responsiveness, attentiveness, and cooperation throughout the session. No signs of hallucinations or
dissociation were observed, and her perception seemed normal

V. Test Interpretation
C.M. has a normal relationship with her mother, but they are not close. Unlike most children
who can share essential or small things with their mother, C.M. stated that they are not like that. One
of her main reasons is that her mother's mindset and beliefs are opposite to her beliefs in life, and
because of that, they often have misunderstandings. However, despite that, the client expresses her
love and admiration for her mother, as she has seen her mother do her best to raise and love her in
her own way. C.M. sees her mother's flaws but accepts and tolerates their differences. On the other
hand, C.M.'s relationship with her father is hostile, and they are not on good terms. She expresses
frustration about her father, feeling extreme hostility and contempt towards him. The client feels
detached from her father and wishes that he would change. C.M. mostly shows no compassion
towards her father. When it comes to her family, C.M.'s relationship with them is not good. She stated
that her family feels toxic because, even as a child, she saw things that made her aware that they are
not a loving family. Unlike most families she knows of, C.M. feels a lack of solidarity, which has resulted
in difficulties and her negative attitudes toward her family.

C.M. thinks highly of women, with women who have their unique beauty and intelligence, but
she stated that she sometimes feels intimidated by them; she has enormous respect for and thinks
highly of women. The client only has minor or superficial criticism about the subject. C.M. appears to
have given up achieving reasonable sexual adjustment, as she stated that she sometimes feels
awkward and self-conscious, and because of that, her self-esteem decreases. Moreover, she views
marital relationships as scary and suffocating, which may be the result of her problems with her
parents and how she witnessed them before.

C.M. has a positive relationship with her friends and considers them as family. They support
and help each other through good and bad times. Although the client's circle of friends is small, their
bond and trust for each other give her warmth and joy, knowing that she has them. C.M. enjoys a
positive interpersonal relationship with her friends and expresses mutual understanding with each
other. Regarding her ability to supervise, C.M. feels comfortable and well-accepted. She believes that a
good leader should be open to ideas beyond just organizing tasks. A good leader must also have the
courage to challenge their ideas or beliefs for a positive outcome. C.M. thinks that people she
considers superior possess more knowledge and intelligence and respects them greatly. However, she
sometimes feels intimidated when her superior is around, causing her self-esteem to decrease. The
client has mild difficulty accepting authority. Furthermore, C.M. has a positive relationship with her
colleagues or classmates and can freely express her emotions and thoughts when it comes to academic
output required. She and her colleagues share good mutual feelings.

C.M. is afraid of being left alone and worries that she might harm herself if she has no one to
confide in. She longs for someone who can understand and love her for who she is or not doing her
best when it comes to something she feels passionate about. The fear of self-assertion is common, but
not pervasive. The client feels guilty about not pursuing the things she loved as a child, and still hopes
to do them now. She also loved someone who was toxic, even though she knew it wasn't right, and
spent a lot of money on them. The client has regrets about the past and is mildly disturbed by her
inability to control her troubles. And expressed her belief in her ability to achieve her dreams.
However, she also admitted that it is normal to feel like giving up at times due to personal struggles.
Despite this, she does not let these feelings consume her and she strives to maintain a positive attitude
towards others. She believes that she can make people happy and enjoyable to be around, even
though she sometimes has trouble with her own problems. One of her main regrets in life is not being
brave enough to try new things when she was younger. She wishes she had been more adventurous
and taken more risks. The client has had no significant disturbances in the past and has a positive
outlook on the future. She has promised herself that she will do her best to achieve the life she has
always dreamed of, to be happy and content, and to improve and love herself more. To always
prioritizes herself and the things she loves and seems to be confident in achieving her goals.
