I, LAILANIE L. RAFAEL, of legal age, married, Filipino Citizen,
presently residing at Runruno, Quezon, Nueva Vizcaya, after having been
sworn in accordance with law, hereby depose and state: THAT--
1. I am currently holding the position of Alternative Learning School (ALS)
Literacy Facilitator at Community Relations Department of FCF
Minerals Corporation.
2. On August 30, 2023, I have entered incorrectly the date of births and
the correct spelling of the name of the ALS Learners in the Learners
Information System (LIS) of the Department of Education, to wit;

b. Akiangan, Jestony Ducusin
c. Lucbuban, Murphy Oras
d. Daling, Narsilyn Laguitan

wit;

5. I am executing this affidavit to attest the truthfulness of the foregoing
facts and for any purpose this may legally serve.
