With reference to your application for admission to Adeleke University, Ede, and consequent upon your academic
qualification and performance at the interaction exercise, it is my pleasure to inform you that you have been
offered provisional admission to study B.Sc. Anatomy in the Department of Anatomy, Faculty of Basic Medical
Sciences of the University.
The admission which is for 2023/2024 academic session is subject to satisfying the following conditions during
registration.
1. Presentation of originals and five (5) copies each of stated documents as follows:
a. O/Level result which must include five (5) credits at not more than two (2) sittings in the compulsory
relevant subjects for your course of study.
b. Birth certificate or sworn affidavit of date of birth.
c. Letter of undertaking of good behaviour from your parents/guardian and a religious leader.
d. Five (5) copies of your recent passport-sized photograph.
e. JAMB-UTME slip of a score not below the national cut off score for university admission.
f. Certificate of origin from State or Local Government.
g. Adeleke University Admission letter sent to you via email (or downloaded from your admission page).
h. Duly completed acceptance form.
2. Payment of non-refundable aceptance fee of Fifty Thousand Naira (₦ 50,000.00) only, which will eventually
be part of your school fee, through the university admission portal within two (2) weeks of receipt of this
letter.

The composite school fees of which covers Tuition, Osun State. I.T. Training, Accommodation, Feeding,
Registration,
Student Association Fee, Medical Examination and Parent Consultative Assembly annual due is payable
in four (4) installments within the academic session. The first installment of the school fees must be paid online
through the university students' portal at https://eportal.adelekeuniversity.edu.ng/payments/fees/start and
clearance printed for presentation at the gate for admission into the campus upon resumption.

UNDERGRADUATE ADMISSION ACCEPTANCE FORM
Acceptance of provisional Admission and undertaking to satisfy all admission requirements into Adeleke
University, Ede.
1. That, upon my application for admission into Adeleke University, Ede, and satisfactory performance
in the various entrance examinations/screening, I have been offered a provisional admission to study
B.Sc. Anatomy (Faculty of Basic Medical Sciences) with effect from 2023/2024 Academic
Session with a provisional admission letter dated October 19, 2023.
2. That, I accept the said provisional admission and that my registration is based on satisfying the
minimum requirements/conditions attached to the admission for my course before the date of the
Adeleke University, Ede, undergraduate matriculation ceremony for the 2023/2024 academic
session as contained in my admission letter.
3. That, I fully understand and unconditionally accept that if I fail to fulfill the minimum
requirements/conditions in paragraph ‘2’ above, my said provisional admission into Adeleke
University stands withdrawn and or revoked by the University and forfeited by me.
4. That, I irrevocably acknowledge and accept that in the event of my forfeiting the said provisional
admission in the above-mentioned circumstances, I shall not be entitled to any refund whatsoever
from Adeleke University, Ede, for any payment made by me or on my behalf to the University.
5. That, I fully understand and accept that my admission to Adeleke University is on probation for the
first academic session. I am expected to maintain good academic standing among competing
colleagues in order to move to the next level in the programme. If found lacking, I shall be ready to
withdraw from the programme and move to any programme that may be recommended to me.
