General Guidelines
A. Assigning Logins on the Dashboard:
The database containing all the field investigators' and Junior Medical Officers' information
will be uploaded onto the dashboard beforehand. The state coordinators are required to Assign
each of them the respective survey schedules they are accountable for in each PSU. For
instance, Investigator A, accountable for the HH and IYCF surveys, needs to be assigned to
these two respective schedules in PSU X or Y, or both.

B. Accessing schedules on the tab


Once the assignment procedure is completed, investigators will be able to access the assigned
schedules on the tab. Each field investigator, Junior Medical Officer, and lab technician specific
to each state is given a five-digit unique code (investigator’s id). They will obtain these codes
from their respective state coordinators. The investigator's code will serve as the login
credentials to access all the survey schedules on the tab.

Quick overview of APP-based Survey procedure


In each PSU, there are nine schedules to be surveyed: one Enumeration schedule, one Household
schedule, and seven Individual schedules [IYCF, KAP, FIES, Anthro, Biomarker, 24H-recall,
and TDS].
Step 1: Mobile Application on the tab
Begin the survey by entering details into the enumeration schedule within the tab using the
provided credentials.
Step 2: Dashboard
After completing the enumeration for all selected households (HHs) in a PSU, access the
Dashboard portal to generate a sample of 30HHs based on systematic random sampling.
Simultaneously, samples for a repeat diet may also be generated as well.
Step 3: Mobile Application [Household Survey]
Once the 30 sampled HHs are generated, access the HH schedules in the tab using the provided
login details and fill in all the required information.
Step 4: Individual Surveys
Once one HH is filled up and submitted, the HH members' information such as name, age, sex,
etc., will automatically populate in each individual schedule (IYCF, KAP, Anthro, Biomarker,
24H-recall, FIES, etc) for that particular HH. These individual schedules will now be accessible
for the survey using the provided credentials. Begin the individual survey.
Step 5: repeat diet survey (2nd and 3rd recall)
Once the 1st recall is finished, access the repeat 24-H diet schedule through provided login
credentials and conduct the survey for the 2nd and 3rd recalls.

Path: Assign >> Assign Survey to Evaluator


1. Click the Tab ‘Assign’
2. The Page ‘Assign Survey to Evaluator’ opens
3. Select the State
4. Select the District
5. Select the PSU [multiple sections]

6. Select the Survey>> Relevant Survey schedules are listed [multiple sections]
7. Select the Evaluator (Type the exact spelling of the Investigator in the search bar)
[multiple sections]
8. Click the ‘Assign’ button
9. System displays the Success message to assure that the Evaluator is assigned to the
Survey
10. Only assigned Evaluator can take the relevant Survey
11. The newly ‘Assign Survey to Evaluator’ is displayed in the Grid below the page.
12. The ‘Assign Survey to Evaluator’ r details can be modified using the ‘Edit’ Button
13. The ‘Assign Survey to Evaluator’ details can be deleted using the ‘Delete’ Button

III. VIEW ASSIGNED SURVEY DETAILS

Path: Assign>> View Assigned Survey Details


1. Click the Tab ‘Assign’
2. The Page ‘View Assign Survey Details’ opens
3. The Page displays the details like unique generated Survey ID, Survey name, Officer
Name, State Name, District Name and PSU Name
4. Verification of successful assignment to investigators or PSUs can be done by searching
for their names in the search bar.
5. Click the icons of Excel sheet or Pdf at the top right of the grid which will export the
Assigned Survey Details to Excel Sheet and PDF icons

Generating Sample Households

Path: Generate Random HHs >> View Generate Random HHs Details
1. Select the State
2. Select the District
3. Select the PSU
4. Click on the "Generate" tab
5. The sample 30HHs will be generated
6. Click the Excel sheet icons at the top right of the grid to export details of 30
sampled households

Details
Follow the previously outlined procedure by selecting the state, district, and PSU to
generate samples for the repeat diet survey.

Consent
To obtain consent for the survey, click on
the downward arrow

If the household (HH) gives Consent,


select 'Obtained' from the dropdown menu,
and then tap on the ‘Start Survey’ button
to initiate the survey.

If the household refuses consent select


'Refused' from the dropdown. This action
will disable the 'Start survey' option.
In cases of a locked door, select the ‘door
lock’ option from the dropdown. But in that
scenario, the investigator will make two
additional attempts to check if the HH
member(s) are available. If someone is
present, select the 'Available' option from
the dropdown to re-enable the 'Start
survey' option. Click the button to initiate
the survey.

Q1: Select the Type of Primary Sampling


Unit (PSU) from the dropdown menu.

Q2-10: From question 2 (State) to question 10


(Date of Interview), all the information will be
auto-populated from the Enumeration
schedule.

Add the Interviewer's Name and Code in the


provided box.
For the Interviewer's Code, the first two initials
of the state name should be in BLOCK letters
(e.g., TS). And THREE digits should be
numeric

Press "SAVE AND NEXT" to proceed to the


Household Roster

HOUSEHOLD ROSTER
At the top of the HH roster, the demographic details
of the household will be displayed.
Fill out the HH roster for each family member
one by one accordingly.

Do you know the date of birth: If ‘Yes’ is


selected, the DOB box will appear with

DOB: Tap on the Year


(top left corner) to select
the Year, then choose the
month and date, and press
SET (bottom right).

Date of birth? Year, Month and Date will be auto


populated in the three boxes

Date of birth? If ‘No’ is selected, the DOB box
will disappear, and the specific age can be
entered manually.
Note: In case the month is not identified, add the
estimated year in the Year box and add a '0' in
both the Month and Date boxes. Adding a value,
at least '0', is mandatory to proceed.

A comprehensive list of subcategories for major


occupations is provided. Click on the Blue icon to
explore the corresponding subcategories, from
which the major occupation can be selected.

Fill out the remaining fields and press +ADD to


proceed to the next family member.
After inputting all information such as Name, Age,
Occupation, etc., the prominently highlighted information
will appear in the grid box below.

Fill in the details of the remaining household


members following the same procedure.
Once information has been added for all the
members, press SAVE & NEXT.

EDITING HOUSEHOLD ROSTER

Editing the Household Roster:


Once the Household Roster has been
completed for all household
members, if there is any information
that requires correction, tap on the
Click on the ‘EDIT’ option at the
bottom of HH roster page.
The Household Roster EDIT page
will be displayed.

Select the Individual from the


dropdown list for whom information
needs to be corrected from.
The previously entered information will
be displayed.
Make the necessary edits in the
respective fields.

After making the corrections, click on


the ‘EDIT & SAVE’ button.

Repeat the same procedure for any other members if needed.


Once all necessary editing is done, click on ‘NEXT’ to proceed to the Socioeconomic
Particulars page

NOTES
 The DoB field only accepts completed years and months, without rounding up running months.
The maximum allowed months are 11, with 12 months counting as an additional year.
 Marital status, occupation, and annual income are displayed only for individuals aged 13 years
and above.
 Physiological status: The physiological status dropdown for females aged 15-49 exclusively
displays options 1, 2, and 3. For females aged 50 years and above, only option 9 is displayed.
 For individuals aged 0-23 months, the dropdown displays options 4, 5, and 6.
 For females aged 2 to 14 years and males above 2 years, the dropdown displays option 9 only

PREVIOUS. Please note that without SAVING
the data of the current page, clicking the 'Previous'
button will cause data loss.

Q55: Physical activities

Under Q26, there is a list of time slots


with one-hour intervals covering the
last 24 hours. For each hourly slot, a
list of various physical activities is
provided on the right (below image),
scroll down for additional options.
The investigator is required to select
one activity at a time from the
dropdown menu. This selection
should correspond to the activity
undertaken by the participant during
that specific hour.
If a particular activity is not listed in
the dropdown, please choose the
‘Other’ option. This will prompt a
specification tab to appear, where you
can specify the activity in question

While filling out all the fields, please review the information once and then proceed to click
on the ‘Submit’ button to finalize the data submission.

 For children <12 years, only height, weight, MUAC (Mid-Upper Arm Circumference) will be
displayed in the schedule.
 For individuals ≥ 12 years, height, weight, MUAC, waist circumference, fat fold thickness,
blood pressure will be displayed
 For all ‘PREGNANT’ women height, weight, MUAC and blood pressure will be displayed.

For every measurement,
except for FFT, three
readings are available.

Duration: When click on the
dropdown arrow, a calendar
will appear, fixed to the last

and press SET (bottom right).

If 'Yes' is selected for option ‘Nil’, the

Introduction
The study’s main objective is to map the Diet, Nutrition & Health Status and Biomarkers among
urban and rural populations across all 36 states/UTs of India, using a standard and uniform
methodology.

Principle

Assessments of Food and Nutrient intakes of individuals by 24Hour Recall method of Dietary
Assessment on the “Reference Day” (capturing the previous days Diet).

Aim

To major collect information on quantitative dietary intakes of the population.

Objective
To collect the information about consumption pattern of individual family members in the
previous 24Hour Recall(reference day), including the “Away from Home Foods” or “outside
food”, and individuals participating in food security Programmes sponsored by
Governments/NGO’s like Saksham Anganwadi and Poshan 2.0 (ICDS) / PM POSHAN
(Poshan Shakti Nirman) Scheme (MDM) / Public Distribution System (PDS) etc.
Training of the Investigator.

 The investigator has to be well trained with the Aims, objectives and methodology of the
study.
 The investigator has to be trained in all the aspects of the study and a special focus made
on the concerned duties to be discharged like methodology of 24Hour Dietary recall.
 The investigator has to attend regular reorientation training sessions periodically.

Responsibilities of Investigator

 The Interviewer & team (along with ID card) need to introduce themselves to the head of
the HH and explain the aim, objective and the purpose of the 24Hour dietary recall.
 The interviewer needs to explain why only this Village, HH has been sampled and the
procedure for the selection.
 Seeking permission from the Head of the household (who is earning and mainly
responsible for the maintenance of the HH and takes decisions on behalf of the HH) is the
first and foremost step for any community surveys.
 Take the consent form signed from the HH head. After taking the consent from the Head
of HH, the investigator/nutritionist needs to ask for the person who cooks and serves the
food to the family members and seek his/her consent.

 This person is the Respondent with whom the 24Hour recall diet survey will be
conducted. The respondents consent and availability should be asked.
 The investigator should be very polite in his/her words with the respondent.
 The purpose of the visit and the importance of the diet survey must be explained to the
respondent before initiation of the survey, so as to elicit factual information regarding
their previous day’s dietary intake.
 The investigator must be familiar with the local measures, weights, volumes, names of
recipes & ingredients and commonly consumed outside foods.

Login: The investigator administering the schedule should login with the assigned login
credentials and also mention his/her Name in capital letters along with the given unique ID for
the investigator.

Remaining data regarding cooked and individual intakes is obtained under the following

 Individual intake – The names will be displayed as per the HH members, so record the
food /recipe intakes according to their consumption as the respondents answers.

meal. If member of the particular family does not participate in 24 Hour recall his/her

Data regarding Beverages – Alcoholic intake have to be assessed for every individual of the
HH.

The interviewer has to select each individual from the household. the Name and age and sex is
auto populated from the respective HH schedule.

 Select whether the individual taken alcohol or not and for each and every individual
family member, need to assess whether they have consumed any supplements are not.

 Type of alcohol need to be selected from the drop-down menu

 Enter the quantity of the alcohol consumption in ml.
