Instructions

1. After filling in the form, put your signature on the designated places.
2. Attach 2 (two) recent colored Passport Size photos with the application.
3. Do not write anything in the shaded areas designated “For Office Use Only”.
4. Payment of Tk. 2,500.00 (Two. Thousand Five hundred taka only) can be made online
5. Attach your Statement of Purpose and Research Intent with the application form .
6. The filled-in application form along with attested photocopies of Certificates, Transcripts/Marks sheets, Work
Experience Certificates, Equivalence Certificate (if required) & the proof of payment will have to be emailed to the
following email address: [email protected] (Last date of submission of Application: November 15, 2020)
7. Add additional paper, if necessary, to complete a section.
8. After submitting this form, collect your Admit Card from DBA Program Office on the assigned
delivery date.
