Dr. Kawther Ali

Assistant Prof. of Nursing Administration
LESSON OBJECTIVES:
By the end of the lecture the student will be able to:
Define the clinical information systems?
Identify the following systems as a components of clinical information
systems: nursing information system, order entry & monitoring
information systems?
Identify the functions & benefits, and advantages & disadvantages of
various types of information systems used within the healthcare
institutions?

The Clinical Information Systems

The clinical information are large,

computerized database management

systems that support several types of

activities.

4
The Clinical Information Systems
➢ The clinical side of health information systems include
provider order entry, results retrieval, documentation
such as lab results, and decision support across
distributed locations.

➢These systems can assistant the healthcare provider to

access data that are used in developing patient’s plan,
implementing, and evaluating care.

Components of Clinical Information Systems

See figure 6-1 page 115.

5
Application of Clinical Information Systems
• Drug control – medication dispensing/ordering
• Patient Assessment
• Evaluation of patient care
• Clinical Pathways
• Monitoring patients
• Staff coordination
• Tracking patients in hospital
• Donor databases
• Tracking Devices & Monitors
• Imaging equipment

6
Components of Clinical Information Systems

Nursing
Information Systems

• Supports the use and

documentation of nursing
processes and activities.

• Provides tools for managing

the delivery of nursing care.

7
Nursing Process
• Communicate & coordinates
care with ALL other clinical
disciplines
• Coordinate discharge
planning, education &
teaching, transition of care
• Manages ALL information
related to the nursing
process and patient.
Components of Clinical Information Systems
Goals of Effective Nursing
Information Systems

▪ Support the way that nurses function,

▪ allowing them to view data,

▪ collect necessary information,

▪ provide quality client care, and

▪ document the client’s condition and care that was given.

Components of Clinical Information Systems

Goals of Effective Nursing

Information Systems

Support and enhance nursing practice

through improved access to
information and tools such as
online literature database, drug
information, and hospital policy
and procedure guidelines.

10
Components of Clinical Information Systems

Monitoring System

Monitoring systems are devices that automatically

monitor biometric measurements in critical care and
specialty areas, such as cardiology and obstetrics.

Theses devices send information to the

nursing documentation systems.

11
Nurses integrates data from:

• Hemodynamic devices
• Mechanical ventilators
• Bedside testing devices
• Physiologic monitors
• Arrhythmia monitors
• Hemodynamic monitors
• Observation from direct patient assessments
Arrhythmia Monitor
• Computerized monitoring and analysis of cardiac
rhythm
• Basic Components
– Sensor
– Signal conditioner
– Cardiograph
– Pattern recognition
– Rhythm analysis
– Diagnosis
– Written report

Order Entry Systems

This process is known as Computerized Physician, Provider or

Prescriber Order Entry (CPOE).

CPOE is a system that allows providers to place specific

orders electronically through the computer.

It is replacing verbal, written and fax orders.

15
Process of CP Orders are:
Medications and treatments, laboratory and radiology investigations,
diet type are entered into the computer and directly transmitted to the
appropriate areas such as the pharmacy, chain management,
laboratory, radiology, kitchen and other service areas.

16

Order Entry Systems

These orders are entered by

healthcare professionals such as:

physicians, nurses, physical

therapist, radiologist, laboratorist

and other providers.

Components of Clinical Information Systems

Goals of Order Entry Systems

Eliminate errors related to illegible handwriting and

transcriptions.

Checks orders for accuracy and completeness.

Speeds the implementation of ordered diagnostic tests

and treatment modalities.

19
Components of Clinical Information Systems

Goals of Order Entry Systems

Decreases adverse drug events.

Enhances staff productivity.

Saves money.

Promotes safety.

Improves outcomes when used in conjunction with

evidence based practices.

20
Components of Clinical Information Systems
Advantages of Order Entry Systems

✓ Enhances safety through the incorporation of built-in

reminders and alerts.
✓ Help the prescriber to select the most appropriate
diagnostic test or medication for a particular patient as
well as the appropriate dose and form.
✓ Offers order double checking within a specified time
frame.

21
Components of Clinical Information Systems
Advantages of Order Entry Systems

✓ Reflect the current state of each order e.g. pending,

cancelled, completed.
✓ Allows user to see a comprehensive list of client’s orders
at any point in time.
✓ Offers mechanism for the entry of charges for a
procedure once it has been completed.
✓ Ensures that orders are legible.

22
Components of Clinical Information Systems
Disadvantages of Order Entry Systems
It may create new problems and errors related to new
work processes.
It needs time for training personnel.
Unavailable with computer downtime *
Harder for clinicians to use who are computer illiterate
Clinicians may put in orders in CPOE incorrectly *
Some physician resistance
Some physicians do not like change that is associated
with using CPOE and the electronic health record **
* (McGonigle & Mastrian, 2012, p. 219).
** (Wood, 2013). 23

• Real time locating systems (RTLS)
have the capability to reduce the
amount of time nurses and other
healthcare providers spend
searching for equipment and
supplies by making them easily
trackable.
