Lesson Learned Title: Tie Rods for Anchor Piles at MYDP project
Background (What/Where/When/How?)
Context:
During Material Receiving Inspection of Tie Rods and associated accessories to be used in the Secant Pile wall
and Relieving Slab pile construction at SARCO Site Store, Material received at site from October 2018 to January
2019 supplied by approved supplier M/S. Dextra Splice Ltd. for Maritime Yard Development Project, it was found
that the Tie Rods accessories’ Grade is less than the Grade of the Tie Rods (combination of 500/600 N/mm2 Tie
Rods and 355/600 N/mm2 Tie Rod Nuts) as well as the accessories are not Hot Dipped Galvanized. However
that in section 4.3.2 of Project Specification for Secant Pile wall and relieving Slab Pile Construction PB4784-
T203-MA-SP-4937 Rev-07, it is stated that “Nuts for Tie Bars shall have a Grade of Steel at least equal to the Tie
Bar, and all the components of Tie rod system shall be hot dipped galvanized.
Cost/Schedule/Safety/Quality impact:
The defective Material has widely affects the schedule and quality of the project. All quantity of components were
quarantined until a detailed design report will be submitted and reviewed by Company/Designer and other
additional test required for analysis are completed. Affects the installation schedule due to delayed release of
materials.
Investigation/Root Causes (Why did this happen? If positive experience, what were the benefits?)
Verification of documents related to the submittals of Tie Rod materials and accessories found that specific
requirement is stated in the Purchase Order. While the manufacturer had delivered mixed materials with
different grades.
There are lapses on the Contractor’s material receiver since the materials have been delivered at site almost
four months ago without review of material test certificates/quality documents and thorough inspection of the
material itself that caused late discovery of non-conformances.
Delayed in submission/provision of quality documents by the supplier/manufacturer.
