This checklist is an official document approved by UV Gulf. Signing this document will prove that the work has been accomplished and the client has taken services
from UV Gulf under official approvals from both sides. Cancelation of checklist must goes under UV Gulf management approvals before signing. Any alteration or
modification on checklist has been made without designated personnel signature and stamp or official approval from UV Gulf will be consider rejected.
