A. Organization of Entries

the manuscript is arranged in the following sequence:

2. Text
The text of the thesis is divided into chapters. The text section is
paginated using Hindu Arabic numerals, beginning with the number 1
on the upper right-hand corner. The page number does not appear on
every beginning page of the chapter. Each chapter must begin on a
new page.

3. Back Matters
Reference (Required) continue numbering in Arabic numerals (for
Undergraduate) and Roman Numerals (for Graduate/ Masteral)

Appendices (Required) Continue numbering in Arabic numerals

B. Detailed Description of Each Section of the Manuscript

1. Front Matters
a) Fly Leaf (Blank page)
b) Title Page (See SAMPLE FORM 1: TITLE PAGE)

This p a ge contains the Thesis Title, School, Institution.

Program of Study (Degree), Author/s and Date.

The title page is counted as page i (lower-cased Roman

Numeral), but unmarked. The pagination is reflected in the Table
of Contents.

The title of the thesis should be in uppercase and

centered. It should be presented in descriptive words that will
ensure electronic retrieval.

If possible, use word substitute for formula, symbols

and subscripts/superscripts.

The final line of the title page is the month and year in which
thesis is approved by the Oral Defense Panel.

c) Approval Sheet

This form must be typed or printed on a laser printer and

must contain the seal of the Research Center. This can be
secured from the SGS Office. (See SAMPLE FORM 2:
CERTIFICATE OF PANEL APPROVAL)

The Approval Sheet must contain the signatures of all

the members of the Oral Defense Panel. Date for affixing
the signatures must also be indicated. The signatures should
be original and must be rendered in blue ink.

This is page number ii, but unmarked. The pagination is

reflected in the Table of Contents.

d) Acknowledgment

The word ACKNOWLEDGMENT in upper case and

bold attribute should be centered. (See SAMPLE. FORM 5:
ACKNOWLEDGMENT)

This page expresses the student's' recognition of, and

appreciation for any special assistance made by individuals
that are not mentioned in the manuscript but have made an
important contribution to the study.

e) Dedication

The heading of a one-page Dedication may not

appear on the dedication page, and the text may be
centered and must be in the third person.
 This contains name/s of special people, e.g. family and
friends who are close to the author and who inspire him/her in
completing the work

f) Abstract

The heading for this page is ABSTRACT in

uppercase and bold attributes, centered without punctuation.
(See SAMPLE FORM 3: ABSTRACT)

The body of the abstract, printed in double space, begins on

the fourth line below the heading not more than two pages.

An abstract is a general overview of the research. It

is an essay summary of the thesis; a statement of the experiment
or problem, the procedure followed, the results, conclusions and
recommendations.

It should be a succinct account that allows readers to make

an accurate decision as to whether the full contents will be
beneficial to the reader.

Pertinent places, full names of people and other proper

nouns useful in electronic retrieval must be included.

Symbols, as well as foreign words and phrases, must be

clearly and accurately displayed.

Transliteration for characters other than Roman Greek

Letters and Arabic Numerals as well as accents and diacritical
marks are included.

Use present form in all sentence construction, if possible.

g) Table of Contents

The heading for this page is TABLE OF CONTENTS in

upper case and bold attribute, centered without punctuation. (See
SAMPLE FORM 6: TABLE OF CONTENTS)

The titles of chapters must be listed and worded exactly as

they appear in the body of the thesis. Side headings must not be
numbered.

Titles for the preliminary pages and for all chapters,

except subchapters, are in uppercase.
 The page number of the starting page of each part is
listed flush at the upper right margin.

h) List of
Tables/Figures/Appendices/Illustrations/Symbols/Abbrevi
ations

The headings for these pages are in uppercase and bold

attribute, centered without punctuation. (SAMPLE FORMS 7-10)

All tables and figures are arranged in increasing Arabic

numeral.

Appendices, symbols, and abbreviation are arranged

alphabetically. Greek-lettered symbols are placed before
Roman/English-lettered symbols

Each acronym/symbol is indented and separated from

the definition by a dash line.

Consistency in the inclusion of tables and figures must

be observed; if tables from one chapter are included in the list,
then tables from all the other chapters and the appendix must
be included.

Each entry must list the same caption or title used for a
table in the text, although a long caption may be abbreviated
to the extent of using only the first full sentence or clause.

The most common mode of presentation is to sing -

space within and to double-space between each entry.

2. Text

Although the detailed organization of the text may vary among

academic disciplines, the formatting of the text must be followed
consistently throughout.

All headings and sub-headings should be presented in the

same way in each chapter or major section, in terms of capitalization
placement on the page, and font type used.

No headers or footers giving the titles of chapters, other

sections, or the thesis are allowed.
 Body of the Thesis

The manuscript is organized by sub-chapters.

The chapter number and the title must be in uppercase and

centered Leaved a double-space distance between these items.

The subchapter or subheading must be in lowercase, emboldened

and placed at the left hand corner. Leave a 4-space distance between the
title and the subheading

Numbering for each chapter is in Arabic Numeral (*) and Roman

Numeral (**). The above chapters are just recommendatory. Chapters and
sub-chapters other than stated above may be added.

a. Chapter and Sub-Chapters

The heading for each chapter should be centered without

punctuation. On the second line following this heading is the title of the
chapter. (See SAMPLE FORM; TABLES; CHAPTER; TABLES; FIGURES;
PAGINATION)

The next line starts on the fourth line below the title of the chapter

The sub-titles are flush to the left margins.

The first page of each chapter is counted, but unmarked. All pages
are marked with Arabic Numerals at the upper right hand margins.

All major sections/chapters must begin on new page;

subsections/subchapters need not.

Whenever the heading of a section or subsection appears near the

bottom of a page. it must be followed by at least one line of text, or the
heading should be forced to the top of the next page.
b. Tables and Figures

Tables designated tabulated numerical data used in the body of the

thesis/dissertation and in the appendices. Figure designated all other non-
verbal materials such as illustration, charts, graphs, maps, photographs,
drawing, diagrams, and the like. (See SAMPLE FORM 10)

All tables and figures, if possible should be located such that they
do not run over into the next page. If this happens, subsequent pages of
the illustration must include at least the illustration number and the
notation that it is continued, something like Table 1, cont.

Illustrations may be presented horizontally or vertically. In either

case illustration must fit within the required margins, and page numbers
must be placed consistently throughout the entire manuscript.

Table captions are typed above the table and figure captions are
typed below the figure. If an illustration is rotated, the coordinating
caption must also be - rotated.

For tables and figures copied form the caption of such should
include the source

The captions for table and figures must be identical with those used
in the UST OF TABLES and LIST OF FIGURES in the preliminary
pages

Illustration must be numbered consecutively throughout the entire

thesis, A straight sequence (1,2,3,4 etc.) or a decimal approach
(1.1,1.2,1.3, where the first Digit is the chapter number, and the digit
after the decimal point is the illustration number), may be used.

Photographs, as a figure, must be scanned and convert to an

electronic format simultaneous printing with the pages.
 Below is a sample of a table and a figure; note that the left and
right border line of the table flush with left and right margins .

Table 1. This is the title of the first table. If the title is long such that it runs
down the next line, the first word of the text line must be flush
with that of the previous as shown by this example

illustrations, maps, chart, drawing, diagrams, etc.) in this

3. Back Matters
a. References

The headings are suitable, depending upon

documentation style, including REFERENCES.

The heading must be in uppercase, centered and in bold

attribute. This must be in the same font style and point size at the
rest of the thesis.

Use the American Psychological Association (APA) sixth

edition as your citation and reference guide.

Use the latest publishing date for the book or encyclopedia

and the complete data for newspaper, magazines, or the
internet.

Titles of the book, theses, journals, etc. must be

italicized:

the correct setup including appropriate punctuation and
underlining.

b. Appendix or Appendices
 The appendix provides a place for a supplementary material
that is not necessary for inclusion in any of the major chapters.
Tables too detailed for the text presentation figures, technical
notes, raw data, computer programs, musical scores, sample
questionnaires, schedules case studies are common appendix
materials. (See SAMPLE FORM 12: APPENDIX)

Appendices, presented in alphabetical, order (A, B C,

D,... )and may be single-spaced, appear at the end of the thesis
text only; they do not appear at the end of each chapter.

The font type and point size for the main headings, sub-
headings, and illustration captions of an appendix must match that
of the text.

Appendix material may be also included in compact disc

form, with each compact disc labeled using sticker paper as
follows:

c. Curriculum Vitae

It is a professional bibliography of the candidate that may

include place and date of birth, educational background, etc.

It should be short, concise, written in the third

person, and the same Font type and point size as the rest of
the thesis; must be double space. Use only one (1) page for
undergraduate and one (1) or two (2) pages for graduate thesis
 It is must be include as the last item in the table of
contents.

C. Technical Specification

1. Paper Size and Quantity

8½ x 11" short-size legal white substances 20 book paper must be used.

2. Paper Layout

Paper must be printed/typed in portrait mode. Landscape mode is

discouraged except for purposes of accommodating unusually large tables,
illustrations, and the like

3. Margins

To provide allowance for trimmings during binding and later case in

microfilming and copying, every page of the manuscript must meet these
minimum margin standards:

Top: 1.00"
Bottom 1.00"
Right 1.25"
Left 1.50" (binding edge)

All manuscript materials must fit within these margin requirements

(Including page numbers, tables, figures, and graphs)

4. Fonts and Font Size

A 12 — point Arial/Courier New size is recommended. A consistent font

and size must be observed throughout all sections the thesis except tables. Italic
may be used for quotations and words in foreign language.

5. Spacing

The manuscript, including the abstract, acknowledgement, curriculum

vitae must be double-spaced and should be used consistently throughout he
documents except tables.

6. Pagination
Preliminary pages are numbered consecutively in lower case Roman
numerals; the first page to be numbered is the Abstract page which is marked iii.
The text and back matters are numbered consecutively in Arabic
numerals.
 Page numbers must appear in the same location (upper right hand
corner) on each page except on the first page chapters where the page is
counted but not marked

Page numbers must be consistent with the text in the font size and style;
must also fit within the margin requirements.

7. Text Citation of References

References may be cited by giving the last name(s) of the author(s) and
the year of publication of the reference.

Example:………… was discovered (Longuet and Fox, 1997)

8. Corrections and Errata

No correction in the form of correction tape or fluid, erasures, crash outs,

and the like must be made on the final copy.
9. Printing

All print should be letter quality with dark black characters that are
consistently clear and dense,

10. Binding and Labeling

The manuscript is bound using a hard cover, wrapped in transparent

plastic, in navy blue for Aft green for BSA, red for CTE, and golden yellow for
CIT(Note: Distinction of color depends in each campus)

The hard cover contains the title, name of the author, school, place, date
of approval for binding, all of which are stamped in gold/black foil and set in ',4-
point Arial font with bold attribute for all elements. (See SAMPLE FORM 13:
COVERAGE)

Except for the date, all other items are in uppercase.

Bacterial isolate: Bacillus app. 1 and 2 and ohysomonas sp. 2

study (4) were purified bacteria has the same physical characteristic
based on the reference chart for determination of cultural
characteristics of isolate (appendix A) as in the previous study.
Tables 4.1 to 4.3 listed the observed cultural characteristic of this
research as against that previously characterized.
