TRAINING SESSION EVALUATION FORM

INSTRUCTIONS:
This post-training evaluation instrument is intended to measure how
satisfactorily your trainer has done his job during the whole duration of your
training. Please give your honest rating by checking on the corresponding cell of
your response. Your answers will be treated with utmost confidentiality.

Our trainer has executed the training delivery very well. We were given an
adequate number of CBLM to cater the number of students.. The
activities were too much for us to handle because we were not oriented
what was expected of us. The design and delivery of her lesson was
somehow good.

General Interpretation:
The average rating for institutional evaluation is Very Good and
Satisfactory. The institution has provided its trainees good
accommodation during the period of OJT. We were somehow
supervised and monitored very well and well-guided.

Comments/Suggestions:
The industry partner was very accommodating and promoting. Yet, there
was no good supervision of our progress. They should have a regular
meeting, consultation and advice to us. These will help us in improving
ourselves and for us to know our mistakes in doing the task. The industry
and the school’s supervision was not well monitored. We were not given
what we should do as to how we can more harness our skills.

General Interpretation:
The reason why I gave the average score of 4.45 was that my
overall stay in the industry was good and it harnessed the skills out of
my professional growth. We were given such a wonderful experience and
it made our stay so memorable.
Recommendation:
So far our experience in our training is very efficient , I learned a lot, I just felt
the time allotted for the training is lacking since im a slow learner and I need
more practice
