Phonological Awareness Screening

Introduction: Phonological awareness skills increase a child’s understanding that words are made
up of sounds and ability to manipulate those sounds to create different words. These skills are crucial
foundations for learning to read and spell. The following screening will help you determine which
phonological awareness skills to target with a particular child.
Instructions:
1. Print out the screening form on the next page. Ask the child questions about each skill (use
the examples on the form). Determine whether the child has mastery of that skill and mark
a checkbox in either emerging (little to no understanding), progressing (some understand-
ing), or mastery (completely understands).
2. Print out the labels on the following page. Cut out the label that corresponds to the level
that the child you’re working with needs to practice. Tape this label at the top of the child’s
data sheet or on his/her notebook so you remember to informally talk about this skill when-
ever you notice an opportunity.
