one minute.
Yes, Sir

Let us proceed to our topic.

Very good, class!

3. Presentation
Did you have any idea what is the
purpose of this activity? Yes Joshua It is all about measuring pulse rate sir

Good. Any other answer, Ashley Sir it is about the vitals signs

You are right, for today’s lesson is about

vital signs, you are going to learn the
different ways and methods of vital
signs and the different tools and
equipment being used.

First let us tackle what is vital signs. Yes sir

4. Discussion
What comes into your mind when you It is all about the are measurements of the
hear the word vital signs? Yes body's most basic functions and different
tools and equipment as well, Sir
Excellent!
We can learn how to measure accurately
What else, Ann? and the uses of tools and equipment
definitions and to the different ways and
Excellent! You got it right methods of vital signs, Sir
Okay, let us start our discussion first with
the definition of vital signs.
Kindly read, Yes anika. Vital signs are a group of the four to six most crucial
medical signs that indicate the status of the body's
vital functions, Sir
Good, so we take measurements to assest
whether we are normal or not.

Very good.
Next one? Yes Jam There are four main vital signs routinely
monitored by medical professionals and
health care providers include the following:
 Body temperature
 Pulse rate
 Respiration rate (rate of breathing)
 Blood pressure (Blood pressure is not
considered a vital sign, but is often
measured along with the vital signs.)
Good
Are you familiar with all of these, class? No sir
It is okay, we will be going to learn all of Yes sir.
these examples today.
Body temperature can be measured in
First is the body temperature, kindly read
what is a body temperature. Yes Christine many places on your body. The most
common ones are the mouth, the ear, the
armpit, and the rectum.
Okay good, Body temperature is a vital
sign used to assess the body's ability to
create and expel heat, temperature can
also be measured on your forehead.
Thermometers show body temperature in
either degrees Celsius (°C) or degrees
Fahrenheit (°F). A normal body
temperature should be 36 degrees Celsius
(°C).

Next we have, Yes mark The pulse rate is a measurement of the

heart rate, or the number of times the
heart beats per minute. As the heart
pushes blood through the arteries, the
arteries expand and contract with the flow
of the blood. Taking a pulse not only
measures the heart rate.
Hearth rhythm
Strenght of pulse
Very good!
The normal pulse for healthy adults
ranges from 60 to 100 beats per minute.
The pulse rate may fluctuate and
increase with exercise, illness, injury, and
emotions. Females ages 12 and older, in
general, tend to have faster heart rates
than do males. Athletes, such as runners,
who do a lot of cardiovascular
conditioning, may have heart rates near
40 beats per minute and experience no
problems.
Yes sir.
Understood, class?

How to check your pulse

As the heart forces blood through the
arteries, you feel the beats by firmly
pressing on the arteries, which are
located close to the surface of the skin at
certain points of the body. The pulse can
be found on the side of the neck, on the
inside of the elbow, or at the wrist. For
most people, it is easiest to take the
pulse at the wrist. If you use the lower
neck, be sure not to press too hard, and
never press on the pulses on both sides
of the lower neck at the same time to
prevent blocking blood flow to the brain.
When taking your pulse:
 Using the first and second
fingertips, press firmly but gently
on the arteries until you feel a
pulse.
 Begin counting the pulse when the
clock's second hand is on the 12.
 Count your pulse for 60 seconds (or
for 15 seconds and then multiply by
four to calculate beats per minute).
 When counting, do not watch the
clock continuously, but concentrate
on the beats of the pulse.
 If unsure about your results, ask Yes, Sir
another person to count for you.
Are we clear class? Okay so let’s The respiration rate is the number of
continue. breaths a person takes per minute. The rate
Kindly read, Yes Joy is usually measured when a person is at
rest and simply involves counting the
number of breaths for one minute by
counting how many times the chest rises.

In addition, Respiration rates may

increase with fever, illness, and other
medical conditions. When checking
respiration, it is important to also note
whether a person has any difficulty
breathing.
Normal respiration rates for an adult
person at rest range from 12 to 16
breaths per minute. Blood pressure is the force of the blood
pushing against the artery walls during
Last we have, Yes Justin contraction and relaxation of the heart.
Each time the heart beats, it pumps blood
into the arteries, resulting in the highest
blood pressure as the heart contracts.
When the heart relaxes, the blood pressure
falls.
Sir, it about the pressure inside which is the
blood pressure. If it exceeds upon normal
What is your understanding with that? meaning you have high blood pressure sir.

Excellent. As we proceed. Yes sir.

High blood pressure, or hypertension,
directly increases the risk of heart attack, Noted sir.
heart failure, and stroke. With high blood
pressure, the arteries may have an
increased resistance against the flow of
blood, causing the heart to pump harder
to circulate the blood.
Blood pressure is categorized as normal,
elevated, or stage 1 or stage 2 high blood
pressure:
Normal blood pressure is systolic of less
than 120 and diastolic of less than 80
(120/80)
Elevated blood pressure is systolic of 120
to 129 and diastolic less than 80
Stage 1 high blood pressure is systolic is
130 to 139 or diastolic between 80 to 89.
Stage 2 high blood pressure is when
systolic is 140 or higher or the diastolic is
90 or higher
Importance of vital signs.
Last we have, Yes eljhane These measurements are taken to help
assess the general physical health of a
person, give clues to possible diseases, and
show progress toward recovery.
Exactly! So that we can determine or
identify what kind of sickness and what
kind of medicine we must give to our
patient. Also we could know whether we
are normal or not. None so far, Sir
Any questions, class? Yes, Sir
Is everything clear?
5. Fixing Skills
Will you tell me what are the four main (Mark identifies all the four main vital signs
vital signs? given)

Give me the examples pulse rate. (Christine gave the two examples of pulse
rate)
What is the importance of Vital signs?
(Amanda did stated the importance of vital
Excellent, class! signs)
6. Generalization
Now, what have you learn from our
lesson for today? We define what is vital signs sir
Yes, Jessa!
Okay, very good.

We discussed about the four main vital  Body temperature

signs, Kindly give me those, Yes Amanda?  Pulse rate
Good Job  Respiration rate (rate of breathing)
 Blood pressure (Blood pressure is not
considered a vital sign, but is often
measured along with the vital signs.)
Excellent Amanda!
Lastly, what is the importance of vital
signs? The importance of vital signs is that these
measurements are taken to help assess the
general physical health of a person, give
clues to possible diseases, and show
progress toward recovery.
Very good class

IV. Evaluation
Identify and give the complete definition and understanding of the four main vital signs.
5points each

V. Assignment
At home, do inside chores, watering the plants, washing the dishes, washing your cloths and
sweeping the floor. Afterwards count your pulse rate and submit it to me next meeting.
