BRIEF DESCRIPTION
On October 12, 2022 around 8:20 PM, while cleaning the coffee/ tea mugs at the mug wash
room area located at 13th floor North Wing, the wall tiles beside her came in falling as there
was concrete missing behind the wall tiles and these were not scrapped properly during the
construction phase, this was evidence post the wall tiles had fallen on the HK lady staff
(Ammulu V), causing her head and right-hand fingers to be bruised. She informed the
supervisor about the mishap and in a timely manner, the facility team arrived at the spot and
informed the nursing staff and shifted the HK Staff to Nurse room for giving basic first aid and
after receiving first aid, the injured HK staff was sent to the nearest Hospital for further
treatment at 8:34 p.m.
The facility executive immediately barricaded the area with caution signage as a corrective
action the facility team conducted Internal inspection for all the floor Mug wash rooms dated
13th Oct 2022 and informed the EHS team. A joint inspection walk around was done by EHS
and builder team and the incident were recorded in the CMO tool. Since the building had
completed 4 years the DLP period was not valid, hence the activity was carried out by
external vendor under the guidance of in-house FM teams. The activity of fixing the wall tiles
was completed by 10th Aug 2023
BACKGROUND
1) On October 12, 2022 at 8:20 PM, while cleaning the mugs in the mug wash rooms on
the 13th floor North Wing, unexpectedly wall tiles fell on HK lady staff (Amulu),
2) facility team arrived at the spot and informed the nursing staff. And shifted the HK Staff
Amulu to Nurse room for giving basis first and shifted to nearest hospital for further
treatment
FACTS
 The mug Was room wall tiles were loosened due to aging of the building
 Since the building has already completed its 4years and DLP period was also over

ACTIONS TAKEN
 Done the inspection and replaced all the old tiles with new ones across all the Mug was rooms for re-
spective floors

EVENT RESOLUTION / RECOMMENDATIONS:


Based on the incident all the Engineering team were briefed and instructed on carrying out regular checks of
the rupture and condition of the walls
