official and signed letter from the Company, with date of issue, address, contact details,
position of signatory, stating the nature of the self-employment or business ownership in
Saudi Arabia. The letter must also state the monthly salary (pay roll) drawn from the
company.

2. Self-employed applicants: trade licence registered at the Saudi Chamber of Commerce, official
and signed letter from the Company, with date of issue, address, contact details, position of
signatory, stating the nature of the self-employment or business ownership in the Saudi Arabia.
The letter must also state the monthly salary (pay roll) drawn from the company.
3. Retired applicants: copy of your retirement compensation from social security;
4. Unemployed applicants:
▪ for children and spouses travelling with the sponsoring family member, a family card issued by the
competent Saudi Authorities, translated into English is required in each file. Family cards, birth or
marriage certificates issued by a foreign authority other than Saudi Arabia must be translated and
legalised by the Italian Consular Office of the issuing Country OR by the Consular Authority in the
Saudi Arabia of the issuing Country; sponsorship letter dully signed download from website “FORM

▪ Spouses/parents/siblings traveling without the sponsoring family member should provide the
following documents for their files: employment information of the sponsoring family member i.e.

employment letter, bank statement of the last 6 months (3 months for non-Saudi), family card
original and copy, marriage/birth certificates legalized and apostille showing the parental
relationship plus passport photocopy of the sponsoring family member;

➢ The valid work contract of nanny.

certain circumstances, the document of divorce of death certificate to be presented translated into

Students enrolling in full Italian University Programs must accomplish with all the requirements regulated
by the M.I.U.R. (Italian Ministry of Education, University and Research) to enrol in a full Italian University

• Medical records issued in Saudi Arabia certifying the actual illness and that further
treatment is required in Italy;
• Declaration of the Italian private or public health facility (the latter must be
accredited with the National Health Service) specifying type of treatment, start
date, duration and estimated cost;
• Certificate issued by the Italian health facility confirming the patient has provided
a 30% deposit of the estimated cost for the healthcare requested or,
alternatively, specific regional resolution or authorisation issued by the Ministry
of Health within the framework of humanitarian programmes.
✓ Supporting documentation proving the applicant has sufficient resources in Italy to cover
the outstanding amounts for healthcare, food and lodging outside the health facility, and
repatriation of the patient and his/her accompanying person.
✓ Furthermore, for any person accompanying the patient, health insurance covering a
minimum of €30,000 for emergency hospitalisation and repatriation expenses is required.
The above documents are required for the accompanying person, the hospital must
declare that an accompanying a person is necessary
