SOP FOR WORK PERMIT SYSTEM

1. Purpose
The purpose of this procedure is to ensure that all work is carried out in ORGANIZATION as per the
health and safety principles and necessary precautions and risks have been identified and controlled
before commencing the work and after completing the work.

2. Scope
Permit to work system is applicable to all forms of non-routine maintenance or other activities which
can introduce new hazards during or after their execution.

3. Responsibilities
a. HSE Manager shall evaluate, approve or close the permit after necessary verification.
b. All other departments shall request a work permit before starting the work.

4. Procedure

4.1 Request

4.1.1 Once the job/project has been scheduled and location has been finalized, maintenance
personnel/manager or section responsible for the job should submit the completed “Work
Permit” form to HSE Manager.

4.2 Site Evaluation

4.2.1 HSE Manager and relevant section head should visit the site and evaluate it to determine the
required safety measures before, during or after the execution of job.

4.3 Approval

4.3.1 HSE Manager should recommend health and safety measures (if any) and approve the
permit and project/job leader should be responsible to ensure all health and safety
requirements are met during the work.

4.4 Closure

4.4.1 After completion of job, job leader should ensure that all hazards have been removed from
the location and site is safe for employees and visitors.

4.4.2 After clearing hazards from site, job leader should request the closure of permit from HSE
Manager.

4.4.3 HSE Manager should visit the site and if found safe and satisfactorily completed, work
permit should be closed down and copy of permit should be forwarded to other interested
departments such as accounts and finance if required.

4.5 Durations
SOP FOR WORK PERMIT SYSTEM
4.5.1 Work Permit should be granted for non-routine works for maximum duration of one week
and for larger projects duration should be decided based upon the project
requirements/feasibility.

4.6 Renewal

4.6.1 Due to any reason if the job could not be completed within the initial time period specified
on the permit, time period should be extended or renewed and both the parties should sign
the permit again.
