Dear Parents & Members,

Invitation for Participation in Elite Taekwon-Do Challenge 2024

It is our honour to inform you that Persatuan Choi Jung Hwa ITF Pulau Pinang will be
organizing the Elite Taekwon-Do Challenge 2024, and the details are as follows:

Date : 13th July 2024


Venue : SJK (C) Shang Wu Indoor Hall
Time : 8:00am – 6:00pm
Participant’s fees: RM100 (all events), including Limited Edition of Elite’s T-shirt

Hereby, we would like to invite you to participate in this event. We are sure that your
participations will make this event a very successful and memorable one. Herewith
attached are the entry forms, together with the rules and regulations for your kind
attention and action.

Kindly take note that the closing date for submission of participant’s information and
payment is on 11th May 2024. We kindly urge you to submit the forms as soon as possible
in ensuring the smooth running of the championship.

Important Notes:
1. All participants must submit the hardcopy of the Participation Form(s) to your instructor.

2. Sparring Gears are NOT PROVIDED. Participants must bring their own gears e.g.:
 Sparring Head Gear
 Sparring Gloves
 Mouth Guard
 Groin Guard (for boys)

3. Participant’s fees are non-refundable.

4. Once payment is made, it’s not allowed to change T-shirt sizes. Thus, choose wisely.

5. Organizing Committee has the right to disqualify any participants who does not follow the rules
during the competition.

6. The Organizing committee is not responsible for any loss, damage, injury or death before, during
and after tournament

7. All forms submitted MUST BE accompanied with payments. Registration without payment will
not be entertained.

8. Participant Fees is RM100 per pax (all events).

9. Closing date for submission is on 11th May 2024.

I / the parent/ the guardian of the above name understand and acknowledge that the Organizer has made
and taken all reasonable care arrangement for the mentioned event and shall not hold the Organizer or
the agents, worker or volunteer liable for any negligence, injury, lost or whatever claim arising directly or
indirectly from the above event and, I/ the parent/ the guardian further acknowledge that the Organizer
shall not be liable to additional room charges incurred or any others incidental or any loss or damages
that arise there from and, I/ the parent/ the guardian further agree to abide by the rules and direction of
the Organizer for the purpose of the championship. Accepted for and behalf on the above named.
