1.8. LEADERSHIP AND COMMITMENT

Project Management is responsible for HSE accountability and shall be committed to the
accident prevention program; leadership, participation and cooperation are essential to meeting
our project objectives. Project Management shall lead by example sending a message “safety is
a value” not to be compromised by cost, schedule, and quality objectives. CONTRACTOR
Management (Site Manager, HSE Manager) is committed to perform construction works under
the scope of this PROJECT in a manner that is uncompromising on issues of health, safety and
environment

CONTRACTOR Management will personally practice HSE leadership, demonstrate visible


commitment to the PROJECT’s HSE Policy and strategic objectives, and provide resources to
foster a PROJECT culture that embraces and accepts nothing but optimal HSE behavior. This
will be materialized through the participation and leadership of CONTRACTOR management
in all HSE activities as described through this document such as HSE meetings, HSE
inspection walkthrough, investigation of incidents and any decision making in relation with
CONTRACTOR HSE management system with the support of the HSE management team.

All PROJECT personnel shall actively participate in the promotion of an HSE culture within
the site, for instance, through the participation on HSE activities, motivation program,
preventing any HSE breach by reporting unsafe acts and conditions, and actively participating
in HSE meeting and pre job Task Risk Assessment. COMPANY, CONTRACTOR and shall
strive to promote and achieve a fully committed Safety partnership with all parties involved on
the PROJECT.

CONTRACTOR HSE Policy particularized for this PROJECT can be summarized as follows:

1. Full compliance with HSE Regulations as well as Saudi Arabian legislation and
COMPANY Regulations and Standards.

2. Implementation of a CONTRACTOR Site Safety Program (the present document) to


determine:
- Responsible Health, Safety and Environmental Representatives.

- Identification & Assessment of Risks at work


- Health, Safety and Environmental requirements for all participants in the Works.
- Prevention of incidents/ accidents by means of information and training of all
employees.
- Establishment of procedures prior to the execution of certain activities, as well as
investigation procedures of any incident that may occur.
- Employee safety motivation to prevent unsafe work habits and conditions and
incentive measures.
- The required vigilance to ensure that HSE measures are taken on site at the right time
and following the established procedures, including prompt corrective actions.
- Emergency action procedures.
- Sanitary and medical facilities and assistance for injured workers.
- Preparation of a Site Safety Program and Environmental Plan by each
CONTRACTOR, prior to starting site works to include COMPANY and
CONTRACTOR requirements. Any deviation shall be subjected to CONTRACTOR
prior written approval

1.9. HSE ORGANIZATION

Every reasonable effort shall be made to ensure workers’ health and safety at any moment.
This will only be achieved by means of a well-structured and composed HSE organization.

1.9.1 HSE Field Organization

The following Organizations shall be created in order to implement the Health, Safety and
Environmental System on site:

CONTRACTOR: Main staff members in charge of the CONTRACTOR Site Safety Program
are the following:

-Project Manager
-Operation Manager
-Project Engineer
-Engineer & Supervisor

The success of CONTRACTOR HSE Management System implementation on site shall rely
upon the HSE team and all line management by encouraging an adequate HSE culture among
employees to ensure their participation in the system.

The CONTRACTOR shall provide the HSE manager and each HSE supervisor with a personal
means of communication (e.g., mobile phone).

All CONTRACTOR Personnel shall be evaluated and approved by CONTRACTOR HSE


Management prior the deployment on site. The number ratio of the above HSE Personnel shall
be based on the increase of manpower on site. The CONTRACTOR has the discretion to
increase the number of HSE Personnel according to their analysis.

Take into account that, as per the Construction Safety Manual, Vol. 1 Section 4.5 letter F: The
names and qualifications (e.g. CV/Resume, training certificates) of the safety manager, site
safety supervisor(s) and all field safety officers who will work on the job shall be submitted to
the SAPO for review and concurrence prior to beginning work.

CONTRACTOR personnel shall be evaluated prior to sending to COMPANY for evaluation.

All vacation and leave of absence of the CONTRACTOR HSE Personnel shall be approved by
the HSE Management with proper notification and delegation of responsibility.

Management system on site and inspire an adequate safety culture among employees to ensure
their participation in the system.

Contractor Management

CONTRACTOR shall be responsible for implementing and managing the Health, Safety and
Environmental System on site and, in particular for:

-Allocating sufficient resources for HSE Policy implementation and compliance with
COMPANY requirements
-Ensuring that personnel are competent and have the necessary authority and resources to
perform their duties in a safe manner.
-Encouraging proactive approach to HSE matters by putting in force all possible efforts for a
real involvement of all personnel working on the PROJECT and to improve workforce skills.
-Issuing the CONTRACTOR Site Safety Program (this document) and implement it on site.
-Ensuring the provision HSE training at all levels.
-Having competent HSE advisors at senior level to support the line management throughout the
entire course of the Project.
-Participating in coordination (interface) meetings between COMPANY, CONTRACTORs
that may be working on the same jobsite to coordinate work activities.

2.1 Operation Managers Responsibilities (Contractor Site Management)

It is the Responsibility of the Managers and Department heads:

- To ensure their Staff is competent to carry out their responsibilities.

- To plan and promote training for all levels of employees on a continuing basis.

- To maintain adequate back-up services and resources to enable a safe system of


work, and ensure safe procedures.

- To satisfy themselves that all current SAUDI ARAMCO Legislation Codes of


Practice and Company Procedures are being implemented.

- To ensure all Technical and Safety Legislation and Information is distributed to all
relevant personnel under his control.

2.2 Project Managers Responsibilities (Contractor Site Management)

It is the responsibility of the Project Managers to implement and coordinate with


Project Safety Procedure and SAUDI ARAMCO Safety Policies to actively encourage
and promote Safety.

They shall ensure that all line management is fully conversant with the Company’s
Safety Policy, SAUDI ARAMCO Safety and Loss Prevention Procedures, and to
monitor the effectiveness of Safety performances.

2.3 CONTRACTOR Site HSE Manager Responsibilities

( Site Safety manager/Supervisor)

CONTRACTOR will appoint a HSE Manager to implement the Site Safety Program
and, in particular, he shall be responsible, on behalf of CONTRACTOR, of all HSE
aspects. CONTRACTOR Site HSE Manager will be in continuous communication and
collaboration with COMPANY HSE Manager assigned for the Project, and he will be
reporting directly to the Project Director/Manager. His main responsibilities are:

 Ensure integration of all Health, Safety and Environmental procedures and


standards into the planning and execution of the work
 Assert overall responsibility and accountability for the technical and professional
execution and integration of CONTRACTOR Site Safety Program on Site
 Request the necessary HSE personnel to form his team according to the project
needs.
 Prepare HSE induction and program HSE training programs, in order to ensure all
involved personnel receive adequate and appropriate HSE training according to
their assignment and the PROJECT requirements
 Distribute standards, procedures or any other document that may be required to
organize works safely to all CONTRACTORS
 Ensure that risk assessments and Job Safety Analysis are carried out in order to take
the necessary measures to prevent any risks arising from the construction activities
 Ensure CONTRACTOR provides evidence of the experience, training and
proficiency of employees prior to starting work on site.
 Evaluate CONTRACTOR´s HSE performance on site.
 Investigate methods of minimizing hazards at the workplace and promote interest in
the hazard identification and control strategies
 Prepare and ensure that the relevant reports and statistical information is forwarded
to CONTRACTOR HSE Manager
 Attend CONTRACTOR kick-off Meeting, to ensure that CONTRACTOR is
informed and aware of all Health, Safety and Environmental requirements.
 Attend HSE weekly meeting to ensure the follow-up of the CONTRACTOR HSE
performance.
 Attend weekly progress meeting to ensure integration of all Health, Safety and
Environmental into the planning and execution of the work\
 Attend any other meeting when required.

 Together with CONTRACTOR Supervisors and SAPMT team, pay special


attention to the specific risks of operations that have to be carried out in hazardous
areas or in presence of harmful substances.
 Establish and update the Site Emergency Procedures in all construction phases
 Request the proper installation of an adequate number and type of firefighting
equipment, according to the Site HSE Plan
 Ensure CONTRACTORS compliance with, understanding and implementation with
of HSE Requirements
 Enforce the implementation of the field equipment inspection program by all
CONTRACTORS
 Take part in CONTRACTOR and COMPANY scheduled work area audits or
inspections and implement and record any required corrective action.
 In the event of an accident/ incident prepare accident/ incident reports and
investigations, both for site and governmental bodies in conjunction with
CONTRACTOR
 Promote motivation activities to increase Health, Safety and Environmental
awareness on Site.
 Establish and maintain all Safety, Health and Environmental record-keeping
systems
 Collaborate with permit to work issuer and have available copies of all work
permits required in order to guarantee compliance with their conditions and
regulations
 Distribute 24 hours in advance as minimum, notices communicating all interested
parties of tests that are being performed or warning them about possible new risk
conditions.
 Prepare periodic HSE reports and Final HSE Report, ensuring their distribution in a
timely manner
 Ensure CONTRACTOR understands and implements correctly the Construction
Environmental Management Plan and waste management on site.

2.4 Safety Supervisor Responsibilities

It is the responsibility of the Safety Engineer to:

- Advise all levels of management on current Legislation, Codes of Practice, SAUDI


ARAMCO Safety and Loss Prevention Rules; to keep abreast of all current and new
Legislation, Codes of Practice, appertaining to Company Activities, and to be
giving guidance as to the interpretation to Line Management.

- Monitor Company Safety Procedures and update when required. Make regular site
visits, and prepare report’s on site Safety performances, copies to be given to the
Project Manager, Project Engineers and Supervisors.

- Monitor Personnel Accident Reports, Vehicle and Plant Reports, and Property
Damage Reports. To ensure that all three (3) day periods lost time are reported

- to the Client and Local Authority. Investigate personnel injuries and other incidents
and make recommendations for improvement to prevent recurrence.

- To assist in Safety Training.

- Attend Pre-Tender Site Visits when requested and advise Department on Safety
Requirements.

- Consult with Clients personnel to ascertain any possible hazards, which may be
encountered.

- Liaise with Clients Safety Department to ensure compliance with their conditions,
and to carry out site visits with their Safety Supervisor whenever possible,

- The Safety Engineer will not be assigned any duties nor related to Safety, he will be
a full time Safety Engineer.

- Implement the safety program and procedure.

- Coordinate with Saudi Aramco, and other authorities Safety Departments to satisfy
their safety requirements.

- Supervise Toolbox Safety Meetings.

- Carry out Hazard Identification survey for the whole project.

- Attend various safety meetings.

- Conduct safety audit as required.

- To make sure the works are executed under safe working environments and various
safety procedures as strictly followed.

- Coordinate for the Safety Orientation of the field workers regarding the Saudi
Aramco Safety Rules

- Shall remain present during the execution of the works even on holidays or on
overtime works.

- To staff the Safety Officers / Work Permit Receivers and provide function
supervision as well as performance review on these personnel.

- When unsafe practices / conditions or any safety violations are detected, advise or
direct the supervisor or foreman to correct them.

- Responsible for the preparation, review and maintenance of needed record and
statistical data as required by Saudi Aramco in Weekly and Monthly basis reports.

- He has the authority to immediately stop any segment of work or in some cases the
entire operation whenever he deems it unsafe to do otherwise.

2.5 Project Engineers Responsibilities (Project Engineer)

It is the responsibility of the Project Engineers and Superintendents to:

- Satisfy themselves as to the competency of their supervisory staff.

- Preplan and prepare work procedures and site layouts before the commencement of
contracts, including the involvement of sub-contractors.

- To ensure that all sub-contractors meet their obligations under SAUDI ARAMCO
Legislation, Codes of Practice, and Company Safety and Loss Control, and all
Conditions set-out in the Contractual Documents, and to obtain a copy of the sub-
contractors Policy Statement.

- Keep themselves abreast of current SAUDI ARAMCO Legislation and Codes of


Practice

2.6 Field Engineer & Supervisor responsibilities (Site Supervision)

It is the responsibility of the Engineers and Supervisors:

- To ensure that safe working methods and practices are implemented by those
employees under their control and to ensure that sufficient Plant and Equipment is
made available and properly tested, to enable any operation to be carried out safely.

- To satisfy themselves on the competency of site personnel to carry out their duties.

- To acquaint themselves with the hazards arising from the operations or from
substances used in these operations and convey these findings to the employees and
the general when necessary.

- To make themselves aware of existing SAUDI ARAMCO Legislation, codes of


Practices and Company Safety Procedures, which relates to the operations at

- any time and consult with the Company Safety Department in respect of any Safety
Problems.

2.7 Field Safety Officer Responsibilities

CONTRACTOR Field Safety Officer main responsibilities are:

 Assist the Site HSE Manager in any duty he shall perform.


 Identify and report on a daily basis unsafe acts and conditions.
 Take immediate action to correct unsafe scenarios or initiate corrective measures,
stopping the works if necessary.
 Prepare layout clearly indicating firefighting equipment location.
 Ensure documentation and equipment certificates are available and valid before
entering the site.
 Attend periodical or special HSE meetings as required.
 Assist in the performance of risk assessment.
 Take part in CONTRACTOR or COMPANY scheduled work area audits or
inspections and implement and document required corrective actions.
 Check that the field equipment inspection program has been implemented and tags/
stickers have been provided.
 Check that personal protection equipment and collective means are suitable for the
use for which they are intended.
 Distribute to CONTRACTOR standards, procedures or any other document that
may be required to organize works safely.
 Carry out inspections to ensure compliance with CONTRACTOR Site Safety
Program, CONTRACTOR Site Safety Program, COMPANY requirements and
National and local regulations.
 Support CONTRACTOR HSE Manager to prepare periodic Safety reports, ensuring
their distribution in a timely manner.
 Ensure workers’ welfare is maintained at all times.

2.8 Contractor Personnel Responsibilities

Every employee has responsibility to take reasonable care for the Health and Safety of
themselves and others who may be affected by their actions or omissions at work.

With regards to the duties imposed on their employer or other person by the SAUDI
ARAMCO Regulations, they must cooperate with their employer to enable him to
comply with the regulations.

No employee shall intentionally interfere with, or misuse anything provided for their
safety, or for the safety of others.

All CONTRACTOR employees must comply with COMPANY and CONTRACTOR Safety,
Health and Environmental Standards. Total commitment to safety, health and environmental
goals is a condition of employment

COMMUNICATION PROGRAM

Construction sites require constant communication among different parties involved in the
scope of the contract in order to ensure all workers perform their duties in safe operating
conditions at all times. All necessary resources and measures will be put in place to ensure
fluent communication between all parties no matter what language is spoken.

CONTRACTOR shall ensure that all personnel in the construction site are properly informed
of all HSE rules to be adhered to, all risks and hazards present in their working environment
and all the preventive and protective measures available to avoid any incidents.

Employees shall be fully trained and aware of all the risks derived from:

=Other CONTRACTOR’s work (Simultaneous operations).


=Interfaces with other main CONTRACTORs.

Amongst the measures to guarantee workers are properly trained and informed are the
following;

=Spoken communication by means of toolbox talks or pre-task review of JSA for each crew at
specific crew work face and meetings, among others. This spoken communication may
sometimes be sufficiently informal so as to promote dialogue.
=Training program.
=Written communication includes:

- Motivation resources such as bulletins, posters, slogans, newsletters or flashes,


Information sheets, etc.
- HSE signs and warnings.
- Plot plans located in signboards and key places marking all live lines and especial
dangerous areas. - Plot plan with emergency evacuation routes.
- Board indicating confined spaces throughout the site.
- Useful information in case of emergency, shortest routes to hospitals, etc. all this display in
signboards

HSE MEETINGS

HSE meetings are the best communication mean to ensure HSE performance is being assessed
and followed-up as well as to promote motivation and awareness among PROJECT
management. Moreover, it issued to encourage the workforce to take active part in worksite
accident prevention.

Any involved employee may complete foreseen agendas for any meeting prior to meeting
commencement so as to ensure all relevant issues are dealt with. All meetings shall be recorded
in a written minute of meeting (MOM) which shall be signed by all parties’
representatives and handed over to all attendees when it represents a record for the issues
discussed and decisions made.

Minutes shall include, as a minimum, the following aspects:

-Date and venue of the meeting


-List of persons present and Companies there represented
-Object of the meeting
-Clear record of topics and special issues discussed
-Decisions made and actions agreed (including date and responsible for action)
-Name of person who prepared the MOM and date of issue

Upon COMPANY’s request, CONTRACTOR’s site management, site supervision and/or site
HSE staff shall attend the COMPANY’s HSE meetings as Safe Operations Committee (SOC)
meetings and HSE coordination meetings.

HSE meetings foreseen during PROJECT Construction phase are:

CONTRACTOR Kick-Off Meeting

Prior to commencement of the work on the worksite, a kick-off meeting (KOM) shall be set up
between COMPANY and CONTRACTOR to review HSE requirements and procedures as
well as to ensure that CONTRACTOR’s safety approach will meet all applicable requirements.
Refer to CSM, Vol 1, and Section 4 for further information.

Daily HSE Meetings by Areas

A Daily Safety Meeting by Areas shall be held after the daily HSE inspection between
CONTRACTOR HSE Team with construction supervisor’s attendance.

During this meeting all hazardous conditions and unsafe acts identified during the inspection
shall be reported and reviewed.

Once a week this Daily Safety Meeting shall be a General Safety Meeting to which
CONTRACTOR HSE Manager will necessarily attend. A thorough review of the main issues
addressed during daily meetings will be performed, including but not limited to the following:

-Daily inspections results.


-Hazardous conditions and unsafe acts.
-Incident/accident investigation.
-Follow-up of corrective measures.
-Training activities.
-Procedure and risk analysis, review/update HIP.
-Topics for toolbox talks.

-Working at interfaces planning, coordination between SUBCONTRACTORS planning and
scheduling activities

Weekly Progress Meetings

Weekly Progress Meetings shall be attended by CONTRACTOR Construction Manager and


Supervisors, CONTRACTOR HSE Manager,

The Weekly Progress Meeting is an opportunity to review and follow up the progress of each
CONTRACTOR’s work individually pointing out every aspect of the execution of the works.
HSE matters shall be the first item of revision and analysis during all such meetings. The same
topics reviewed in the Weekly Safety Meeting by HSE Managers and their respective teams
shall be reviewed in this meeting by CONTRACTOR.

Action items and needed corrective actions shall be documented and tracked until closed.
Copies of these documents shall be provided to the SAPO upon request.

CONTRACTOR Coordination Meeting

Weekly internal coordination CONTRACTOR HSE Meetings shall be led by CONTRACTOR


Site Manager and attended by Construction Manager, HSE Manager and Construction
Supervisors.

Special HSE Meetings

In addition to regular meetings, Special Meetings shall be organized by CONTRACTOR for

-Increase the awareness of workers performing the job regarding prevention and protection

and the issues of the talks.

A toolbox talk shall be held every day prior to commencement of the day activities and it shall
last approximately five (5) minutes.

Topics will refer to crew scheduled work activities for that day, to ensure all personnel is fully
aware of the risks and hazards associated to their activities (HIP), the preventive and protective
measures to be implemented. Topics for the toolbox should be logical and sensible. These talks
shall also serve to make sure all new personnel have been properly trained prior to starting the
tasks and to inspect tools and equipment.

Weekly Toolbox Talk

Weekly Toolbox Talks shall be attended by CONTRACTOR employees and workers and they
will be organized and conducted by CONTRACTOR foremen and crew Supervisors.
CONTRACTOR Supervisor will assist to daily toolbox talk in order to supervise the content
and the issues of the talks.

Weekly talks shall be held at the start of every week and shall last, approximately, fifteen (15)
minutes

Topics discussed at this meeting will refer to care and use of respiratory equipment, work
permits when necessary, to be sure if new personnel have been properly trained prior to
starting the tasks, to inform about the new risks/hazards due to coming activities (HIP review),

INDUCTION AND TRAINING

To a large extent, the success of HSE management depends upon educating employees about
their work and site conditions. Each employee must be made aware of his responsibilities,
regulations and procedures relevant to their tasks, the hazards of his work, HSE rules and what
to do in emergency situations.

All personnel shall receive documented training, based on job classification and training needs
analysis, before being permitted access to the work site.

This training shall include

(1) Safety orientation,


(2) Job-specific safety training and;
(3) Job-skills/craft competency training for personnel engaged in specific craft activities (e.g.,
electrical, scaffolding, steel erection, welding, cutting, crane/heavy equipment operation).

Despite the established programmed training activities, during the works progress, different or
additional training sessions shall be organized as deemed necessary.

The efficiency and effectiveness of all induction and training sessions shall be proved by
means of job skill competency testing as part of the courses personnel are taking.

CONTRACTOR’s in-house or third-party training shall include:

- Acquaint the employee with the Work Site, the nature of the job, the hazards that he may
encounter, and the equipment and safe practices to be used to minimize accidents.
-Review the contents of HSE procedures and plans, stressing the sections applicable to the
employee and his job.
-Make employees understand actions to take in the event of an emergency.
-Be familiar with security arrangements.
-Awareness of the importance of HSE matters in order to ensure a safe working environment.
-Advice of the requirements for working safely and that failure to follow the safe practices may
result in disciplinary action including dismissal.
-Provide awareness of the multiple interfaces existing in the project.

CONTRACTOR shall ensure that employees have been trained to ensure competence and safe
performance of their work, and this training shall be appropriate to their type of work.

CONTRACTOR will provide adequate means and language provisions for training and
instruction of their foreign labor force. CONTRACTOR shall overcome language barriers by
Multilanguage posters and booklets. CONTRACTOR shall also overcome inability to read and
write by booklet reporting self-explanatory cartoons.

For all training activities a written record will be kept of all participants. This form will be
signed by all participants. The information of the time, place, attendance and topics discussed
shall be documented and kept for review upon request by COMPANY.

Induction

All personnel shall attend a Plant sponsored safety orientation program prior to Working in the
plant. This will be a maximum of a ½ day program for each Operating Facility. SA Operating
Facility orientation provides safety rules and regulations and Emergency procedures inside an
operating facility.

Apart from this, it is mandatory for all construction and erection employees to attend a
CONTRACTOR Safety Induction Session in a language which is understandable for them
prior to entry to the worksite. No employee will be permitted to work on site or allowed access
to the site without first attending the Induction Session.

As part of the induction, personnel shall complete a test questionnaire to demonstrate their
understanding of the induction. Personnel who fail the questionnaire shall take it over again in
the following scheduled induction session.

CONTRACTOR Safety Induction Session shall include:

-HSE Policy and Construction Site Safety Program.


-Project Standards: basic HSE rules, smoking restriction and locations, traffic rules and
restrictions, heat stress precautions.
-Emergency and First Aid procedures: alarms and notifications, assembly points, Emergency
response.
-Working procedures and standards: HSE signs and warning, housekeeping, noncompliance
and violations.
HSE procedures (PPE, inspections, work permits, color coding, etc.)
-Fire prevention and protection.
-Incident/accident report.
-Waste management and spill prevention and mitigation measures

A Safety basic booklet shall be handed over to all construction employees.

The CONTRACTOR shall also provide a brief visitor induction allowing persons who visit the
site be knowledgeable of HSE rules and required PPE. Visitor shall be all times escorted by a
competent and inducted CONTRACTOR representative

Attendance of designated workers shall be mandatory. These sessions shall be conducted by


qualified personnel.

The relevant topics for Safety training will include but not be limited to:

include “incidents investigation and reporting” training to learn about the method to be
used for incidents investigation, procedure, roles and reporting system.

To visibly verify the training and qualification of employees on site, a sticker with the
name of the course and/ or qualification will be posted on employee´s helmet.

the course. No personnel will be able to undertake work on the Project unless they have
successfully passed the relevant Competency Test.

For individuals who do not pass the relevant Competency Test, refresher training will be
given together with one to one training sessions if the need arises.
The Site HSE Manager will set the relevant pass mark for Competency Tests.

Job-skills competency training

Additional training shall be given to CONTRACTOR employees and worker depends of job
skills competencies to ensure personnel are fully qualified to perform their job properly and
safely as Saudi Aramco and/or Saudi Arabian Government certification/license requirements.

Short Service Employee (SSE)

All new or inexperienced CONTRACTOR workers shall be classified as a SSE for 3 months
and shall be assigned a mentor during this time (experienced employee). HSE Manager shall
inform to CONTRACTOR of all SSE. All SSE’s shall continuously wear a visual identifier
while they are at the job site (a white band on the helmet and it will be removed after three
months). CONTRACTOR shall inform the COMPANY of the maximum ratio of SSEs to
experienced workers that will be present at the job site at any given time. Supervisors shall
observe their SSE’s work performance during three (3) months and provide additional training
if deemed necessary. At the end of this period Supervisor will evaluate the SSE performance,
such as SSE achieve his job in a safe and effective manner.

List of jobs to be performed that require SA approved certification

Operators Certification for Crane and Heavy EquipmentEquipment operators as cranes and
motorized equipment shall be in possession of a valid heavy equipment operator´s license
(heavy Saudi Arab Government licence), if required according to GI 7.025) and the table
below, with the specific equipment stamp for the heavy equipment to which he is assigned.
Also Equipment operators shall have a Saudi Aramco heavy equipment operator’s certificate as
per GI 7.025. The Saudi Aramco Heavy Equipment Certificate shall be valid for three (3)
years. Certifications for all crane attachments and additional crane manufacturers shall also
expire with the expiration of the Saudi Aramco Heavy Equipment Certificate.

CHECKING AND MONITORING PERFORMANCE


(Project Specific Plan/Program)

HSE site inspections are an indispensable mean of checking that the HSE rules are respected
and of maintaining the required HSE level. The aim of site inspections is to provide a snapshot
of what HSE performance at the construction site is like and to flag up the positive points,
points to be improved and points requiring preventive action.

Inspections consist therefore of a checking activity performed by site personnel in order to


check that works are executed in compliance with health, safety and environmental standards
(applicable laws and regulations, COMPANY requirements and CONTRACTOR policy and
requirements). Such aspects will be checked to evidence the degree of effectiveness and
efficiency of the implemented HSE System.

CONTRACTOR shall record in writing all daily observations so as to obtain feasible


information on weak and strong points, so as to ensure continuous improvement on HSE
performance.

Construction Site inspections records shall be used in HSE improvement planning and
communicated and discussed during HSE Meetings. An Inspection Sheet similar to the one
attached shall be used as a guideline to perform field inspections

Specific equipment, tools or machinery inspections will be carried out by CONTRACTOR


according to section “Equipment Inspection Program” and monitored by CONTRACTOR.

Any field observations coming from the inspections described below as well as from
COMPANY Safety Inspections Findings shall be compiled in a centralized system such as
CONTRACTOR construction HSE in-house tool or similar format. This application must
include, as a minimum, all relevant HSE issues inspected as well as a description of
noncompliance’s identified, actions proposed, responsible for implementing the action as well
as proposed closing dates.

Daily HSE Inspection by areas

Daily HSE Inspections shall be carried out by CONTRACTOR HSE Team to point out unsafe
situations or hazardous conditions in order to ensure that no safety or health problems exist and
there are no environmental noncompliances. CONTRACTOR may perform inspection
workarounds together or separately.

All observations, either unsafe acts or conditions, shall be registered and monitor to ensure the
necessary corrective measures are taken. Results shall be communicated to CONTRACTOR by
writing and discuss during Daily HSE Meeting for the correction of the deviations

Weekly HSE Inspection

Weekly HSE inspection shall be carried out by CONTRACTOR HSE Manager jointly with
Field Representative and/or officers. COMPANY is invited to join the inspection round. The
results of the inspections will be registered and analyzed in the Weekly HSE Meetings and in
Weekly Progress Meetings with Construction Management.

These inspections shall analyze not only general aspects but May also focus on more specific
subjects as required:

conditions in order to help in the identification of situations of non-compliance, unsafe
conditions and improvement actions at any time. These observations and corrective actions are
recorded and issued to the responsible supervisors for action as well as addressed in HSE
meetings.

No matter how many and what inspections are scheduled and being performed and managed by
the HSE team, HSE accountability shall be the responsibility of and commitment of all
Supervisor and site management, therefore, they shall:

-Point out to CONTRACTOR any unsafe situation or hazardous condition.


- Request that unsafe situations or hazardous conditions are immediately corrected.
- Inform CONTRACTOR HSE Manager and Construction Manager of any unsafe situation

Construction Supervisors HSE Walk around

CONTRACTOR Construction Supervisors together with CONTRACTOR HSE Manager shall


jointly inspect the site in a regular basis in order to encourage Supervisors in reporting HSE

execution activities.

applied on site. They will be performed according to a dedicated checklist that will include, as
a minimum, the following:

distributed to CONTRACTOR Corporate Top Management, Project Directorate and Site
Management including Construction Supervision.

Site and HSE Management shall distribute the audit report, observations and findings to
Construction Supervision to take leadership in their correction and monitoring.

Site and HSE Management will be in charge of distributing observations and findings to
involved SUBCONTRACTORs for their correction and monitoring.

Third-Party HSE Audit

A part from CONTRACTOR Corporate HSE Audits, an independent third-party approved by


COMPANY Representative will perform HSE audits to verify the implementation of the
CONTRACTOR Site Safety Program (this document) and related HSE documents applicable
at Work Site and make recommendations to improve CONTRACOTR’s HSE programs and
Execution. These audits will start six (6) months after starting activities at worksite and after
that shall be held at least nine (9) months thereafter. The audit reports shall be provided to
CONTRACTOR Project and HSE Management, they will provide copies of all audits reports
to COMPANY Representative.

Site Safety Performance Monitoring

The SAPO (Saudi Aramco proponent organization) will monitor, evaluate, inspect and report
CONTRACTOR job site HSE performance. Performance monitoring will be performed in
accordance with the SA Safety Management Guide, CONTRACTOR Site

Safety Performance Monitoring and/or Project Management’s Project Safety Index (PSI).
CONTRACTOR shall participate in site HSE evaluations and inspections as requested

CONTRACTOR Site Safety Evaluation Form or PSI is considered a violation of the HSE
requirements of the contract. In this case, the SAPO reserves the right to conduct performance
counseling meeting(s) with CONTRACTOR management and/or take other measures in
accordance with SA’s contracting procedures.

Corrective actions shall be implemented by the CONTRACTOR within any SA specified time
period. Corrective actions shall be identified and tracked until completion, with follow-up to
verify proper implementation.

 All SUBCONTRACTORS employees, for execution of any specific activity at Project


Site shall be qualified by Saudi Aramco certification regarding the following positions
mentioned, Saudi Arab Government License, training and experience in his case, and
shall provide the documents to CONTRACTOR Site Manager and all required
certificates proving their skills and qualifications. The list will include: Authorized
Receivers of work permits.
 Excavation competent person
 Riggers / signalers / slingers.
 Crane and lifting equipment operators.
 Electricians.
 Welders.
 Fist aider
 Emergency response team member

Where a specific job function requires SA and/or Saudi Arabian Government


certification/license, these job functions shall be assigned only to properly certified/licensed
personnel (e.g., certified crane/heavy equipment operators per GI 7.025, certified scaffold
supervisors/inspectors per GI 8.001).

Personnel shall maintain the required SA and/or Saudi Arabian Government


certification/licenses in their possession at all times.

SITE SAFETY INCENTIVE-MOTIVATION

Motivation activities shall be developed as part of the Site communication program to prevent
development of poor work habits that always result in an injury rate increase and a quality and
productivity decline. To this purpose all CONTRACTORS shall prepare their Motivation
Program intended for keeping employees aware of the health, safety and environmental issues.

The Program’s objective is to promote site safety and reduce HSE incidents by means of
proper behavior amongst labor force.

The Motivation Program shall lie upon rewarding employees for positive behaviors and actions
performed out of their normal scope of work, always related to incident or injury prevention.
Its effectiveness shall be guaranteed by means of frequent reinforcement and appropriate
program communication and promotion: safety signs, posters, posting of information on
bulletin boards, giving symbolic awards to workers and SUBCONTRACTORS as a team for
excellent performance or behaviors.

The following actions shall be taken to raise HSE Awareness among employees:

 The Project Quality, Health, Safety and Environmental (HSE) Policy shall be
prominently displayed in conspicuously places that are frequented by the PROJECT
personnel.
 Place HSE Awareness posters, slogans and reminders along the construction site and
office/ temporary facilities.
 Erect a board at a prominent location at the entry to the PROJECT site, showing the
HSE performance and achievements.
 Bulletin boards inside and outside CONTRACTOR offices to post general information
on HSE statistics and notices, including longest period worked without a lost time.
 Symbolic award system to reward the worker for excellent performance in the
execution of his tasks and disciplinary actions in case of non-observance of applicable
HSE rules.
 Issuing periodic newsletters on different HSE topics.
 Banners or video films when and where possible
 Publicity shall be in the various languages spoken on the Work Site.

Award System

Site Award System to be detailed and implemented during the first semester of the project
execution between CONTRACTOR Site Management and HSE Manager together with the
COMPANY (if applicable) will include:

HSE Campaign Activities

The CONTRACTOR shall initiate a minimum of one HSE Awareness Campaign every
quarter, depending on the necessity of the HSE Performance Status of the project.

The SCONTRACTORs shall provide all necessary banners, posters, HSE pamphlets, HSE
Alerts, gifts, and any materials necessary to effectively conduct the HSE Campaign Activities.

The purpose of the HSE Awareness Campaign is to motivate the management and employees
to improve the level of compliance and rise up the HSE Standard of the project. Moreover,
carry out the HSE Culture to adjust of the present phase of the project.

INCIDENT – ACCIDENT INVESTIGATION

Incident is an unplanned or uncontrolled event or chain of events, which could or have actually
caused injury, illness, damage to people, assets, the environment or reputation

Accident is any unintentional or unplanned event that causes personal injuries, material losses
and/or damages to environment.

All Recordable and Non Recordable Incidents and Near Misses as well as High Severity
Safety Observations, will be totally investigated. Specifically:

In the event of conflict on the definition of the injuries mentioned above, the definition in GI
6.007 shall prevail.

Environmental Incident (EI) Recordable cases are: On-job Fatalities cases, Lost-Time
Injury/Illness Case, Restricted Duty Injury/Illness Case and Medical Treatment Case. The rest
all under the definition of non-recordable cases.

In order to perform the investigation process, an investigation team shall be created, composed
by the following people, as a minimum:

Construction Supervisor of affected area.


- Mastoura HSE Manager as HSE Subject Matter Expert. He will design the secretary of the
investigation.
- The Site Supervisor as a Chairman of the committee.
- Subcontractor Management Representative.
- Subcontractor HSE Manager (or his designee).
- Injured worker (if possible). - Witnesses (if there are any).
- COMPANY’s representative: as deemed appropriate by COMPANY.

In case of a potential recordable incident (On-job Fatality, Lost-Time Injury/Illness Case,


Restricted Duty Injury/Illness Case or Medical Treatment Case), the investigation team shall
be composed also by CONTRACTOR Site Construction Manager (or his designee), who shall
act as Team Leader.

The investigation team shall focus on finding out all the details related to the incident in order
to identify the direct and root causes that will help to establish corrective actions through a
Loss Causation Model. If necessary, an immediate and informal investigation shall begin
promptly after the incident/accident and results and recommendations for preventive action
shall be submit to COMPANY, prior to the formal and more exhaustive investigation process.

Once the Investigation is finished and root causes have been found, an Incident / Accident
Report shall be issued according to section “HSE Reports & Records” of this document.
Furthermore, an incident/ accident logbook shall be maintained by the HSE Team summarizing
all incidents taken place and it shall be sent periodically (at least once a month or whenever it

Monthly HSE Reports

Accident / Incident Investigation and Report

In order to take the adequate corrective measures, CONTRACTOR shall verbally and
immediately notify CONTRACTOR HSE Manager of any incident/accident occurred within
his work area. CONTRACTOR shall do an immediate oral report to COMPANY
Representative for every case involving job-related fatalities, all on-job injuries and
occupational illnesses, damage to property or equipment (e.g., cranes/heavy equipment,
scaffold, or excavation collapses), all fires and explosions, all oil spills/leaks and gas leaks and
all near misses. COMPANY General Instruction 6.001, Notification Requirements for
Incidents (Including Fires) shall be followed for the notification.

In the event of an accident or incident, notifications must be done immediately and, as soon as
possible after that, investigation should start. It is very important to secure the area as much as
possible so that no evidence is destroyed.

All accidents and incidents shall be thoroughly investigated. The investigation shall be
performed in a timely manner and root causes of the incident shall be properly identify in order
to identify all possible causes so as to implement appropriate corrective actions. These
corrective actions aim the prevention of future situations of the same nature and therefore, the
prevention of accidents. Any investigation performed shall be focused toward finding facts and
causes, never fault or responsibilities.

A preliminary investigation shall be completed promptly by CONTRACTOR HSE Manager or


Field Representative. The main objective of this investigation is to understand what happened,
when and where did it happened, what were the circumstances in which the accident/incident
took place, what consequences did it have and whether there were any injured workers or not.

The same way a Preliminary Incident Report (PIR) shall be submitted to COMPANY within
24 hours of the occurrence of the incident using Saudi aramco CRDD forms.

Further investigation shall be carried out by a specifically designed investigation team led by
CONTRACTOR HSE Manager who will issue a final and complete Final Investigation Report
within 3 days of the incident. The secretary could be the HSE Manager and a HSE Site
Supervisor could be the Chairman of the committee

For major injuries and major damage to property, a written report shall be promptly submitted
to COMPANY Representative using COMPANY´s project management forms. Subsequently
an investigation shall be performed jointly by COMPANY and CONTRACTOR.

CONTRACTOR shall use the Saudi Aramco Project Management Preliminary Incident Report
and Final Investigation Report Form as a minimum including Near Miss report form. Forms
will be provided during the Project Safety Kick-off meeting.

Incidents/accidents investigations and reports shall be organized and carried out according to
COMPANY procedures, G.I.6.003 Incident Investigation and G.I. 6.007 Reporting of
CONTRACTOR On-Job Injuries/Occupational Illnesses

Final HSE Report

After works completion, a Final HSE Report shall be issued and included in the Construction
Final Report. This report will include a summary of every issue occurred during the
Construction phase: whole statistics, incidents and/or accidents investigation reports, best
practices implemented, objectives and targets achieved, lessons learned, etc. Positive actions
undertaken during this period may also be included in the Final Report.

Risk Management

In order to reinforce safe working conditions for all personnel involved in the execution of
works, risk management shall be implemented at all levels of the Project Management.
Specific safety analysis of all individual construction activities shall determine which of these
activities require additional preventive and protective measures so as to reduce risk to ALARP
level.

Risks management methodology shall be implemented at three levels to establish the necessary
preventive measures focused on eliminating or mitigating the risks that can appear during
construction activities:

Hazard Identification Plan

CONTRACTOR shall develop, implement and adhere to a contract-specific approved Hazard


Identification Plan (HIP) The HIP shall list all tasks/activities associated with the contracted
work, potential hazards of each activity and control measures to mitigate these hazards. The
CONTRACTOR’s HIP shall identify all potential hazards associated with the work to be
performed. The HIP shall not be a list of generic hazards. CONTRACTOR Site Safety
Program will include the Hazard Identification Plan specific for the activities under its scope.

updated at regular intervals.

The HIP is the first level of risks management to be performed before mobilization to the site,
to state a general overview of activities, associated risks and measures to eliminate or reduce
them to ALARP levels

Management of Simultaneous Operations

Simultaneous Operations as used in this section refer to undertaking concurrent Construction,


Operation and/or Maintenance activities at the same location, time and/or process system or
equipment. Simultaneous Operations are generally defined as the concurrent execution of two
or more activities which, due to their interaction, result in an increased operational complexity
and enhanced level of risk of the tasks to be performed.

Because of the particular nature of the scope of the contract, special HSE interface
arrangements shall be made by CONTRACTOR, in particular, for non-routine activities so as
to ensure there is a safe working environment any time, any place in the site.

Daily coordination meetings shall be an efficient tool to plan all activities to be performed
taking into account the different interfaces between CONTRACTORS or even with
COMPANY or other CONTRACTORS. All SIMOPS arisen from these interfaces and
interferences shall be carefully planned and performed ensuring appropriate training and
communication exists. SIMOPS studies and reviews will be carried out by CONTRACTORS
in coordination with CONTRACTOR to assess the overall impact and implications of the
simultaneous operation and construction. A specific formal SIMOPS procedure will be
developed later.

A specific Permit to Work Procedure and/or System might be of application depending on the
area where the works are to be executed and the different kind of activities to be performed.

COMPANY and CONTRACTOR reserve the right to proceed to work permits withdrawal
within their respective areas and without prior notice, for safety reasons. Under these
circumstances, works will be stopped immediately, machinery will be disconnected and all
personnel will be evacuated from said area, if necessary. Workers who do not observe safety
regulations may be requested by their companies to leave the worksite.

For typically early stages inside CONTRACTOR’s work permit scope of work a blanket hot &
cold work permit, always with CONTRACTOR and Management approval, may be considered
enough in addition to COMPANY work permit.

Work permits will need cross checking with other existing permits prior to definite approval in
order to ensure no interferences are going to take place.

Any interface work with other CONTRACTORs that CONTRACTOR is leading,


CONTRACTOR work permit system will be of application. For this CONTRACTOR will
provided specific training on WPS to all CONTRACTORs employees involved.

Method of statement

Detailed Method Statements shall be prepared by CONTRACTOR for all work which has risks
to health and safety and environment, as procedures, sequence of operation and the controls
necessary to achieve effective risk management, and efficient execution of the work.

This method statement shall describe the job to be undertaken, individual activities to perform
this job and the sequence of the various activities, equipment, tools, chemicals to be used and
control measures and procedures for each activity including PPEs and emergency resources
required.

The method statement shall be reviewed by COMPANY (when applicable) and


CONTRACTOR Construction Supervisors.

Based on the method statement a Job Safety Analysis shall be performed.

Job Safety Analysis and Task Risk Assessment

In order to ensure safe working conditions for all personnel CONTRACTOR HSE Manager
may consider necessary to perform further safety analysis such as Job Safety Analysis (JSA)

for critical operations or other operations whenever necessary. These JSA are intended to
reduce the risk of undesirable events that may have negative consequences for people, assets or
the environment.

Some works which may be critical and require a further JSA are but not limited to working at
heights, confined spaces, heavy lifting operations, modules hook up, excavations, chemical
cleaning, lock out and tag out, electrical works and blasting. This shall be determined by the
CONTRACTOR HSE Manager who will consider necessary to perform or not JSA depending
on the kind of work activity to be performed, the particular conditions of the activity and
working scenario, existence of SIMOPS and all other aspects that may represent a special
hazard for personnel safety, health or the environment.

The execution of the JSA consists of four phases:

 Detailed description of the work, split step by step


 Identification of potential undesirable incidents for each step
 Consideration of consequences and probability
 Decision on measures

The JSA shall be submitted to and approved by CONTRACTOR HSE team.\

A JSA review meeting will be conducted to ensure all concerns from all stakeholders are
considered.

The Task Risk Assessment shall be duly inform to all worker involved in the task by a toolbox
meeting perform before the task commence. The JSA form shall be signed during this meeting
by all crew executing the activity analyzed to confirm full understanding. A copy of the JSA
shall be available at the work place. In most cases, this JSA will be attached to the work permit
as applicable according to Permit to Work Procedure.

SUBCONTRACTOR may not start work until the site has been inspected by CONTRACTOR
HSE team to ensure that it is safe to start work and that all the requirements contained in the
JSA have been complied with.

To comply with the Safety Management System of RT Refinery & Terminal and Project
Management, work/tasks that are classified as requiring JSA per COMPANY Safety
Management System shall be required to conduct a JSA Review wherein Company JSA form
shall be used.
