Unit 1: Functional Writing: Filling a form


Notes
In this lesson on functional writing, we are going to look at how to fill in a form.
Filling in forms is one of the life skills you will need throughout your life. Indeed, even
as a student you will be asked to fill forms. Here are some examples of forms that
you might have to fill as a student: library membership forms, gym membership forms,
medical forms, club membership forms. The purpose of filling in forms is to provide
information to institutions. Moreover, the information is usually registered in a clear,
orderly and easy format.
Most forms would require you to give your name. Here please note that you are
expected to give your name as it is written on your birth certificate. You would not
usually use your nickname here. Then you would most probably be asked about your
Gender. This is where you indicate whether you are a boy or a girl. You will also be
required to give your date of birth. It is important for you to give this information so
that the institutions know your age. For example, if you are filling a library
membership, the librarian can give you books that are appropriate for your age. In
addition, as a minor, you will also need to have your parents’ permission to participate
in certain activities.
You will also be asked about your address so that letters might be sent to you.
Another important detail that you will be asked to give is your contact number. This
is important as this information will be very helpful in case of emergency.

From the first lines we can deduce that it is an application to become a member of the
Regency DVD Club. We are told to use block letters, which means that we have to
use capital letters when we fill the form. Indeed, when people use block letters they
write more clearly and legibly, so it’s easier to read. Please note that although we have
to write everything in capital letters, this does not apply to your signature. You sign
your name in the usual way.
Our first name is the name by which friends usually call us. As for surname, this is
our family name. for example, if someone is called John Smith, John would be his
first name and Smith would be his surname. Be careful though, in some forms you
may be asked to put your surname first.
The title is another way of asking our gender. Mr. is used for all men; Mrs. is used for
a married woman and Miss is used for a woman who is not married. The contact
number is phone number where you can be reached most easily. The DOB are the
initials of each word Date of Birth. This is where you have to write the date on which
you were born. Some forms might ask you your age directly, thus you will have to
write down how old you are.
The signature is where you sign your first and last name together. Indeed, your
signature is unique. You are the only person in the world who signs your name in that
way. Your signature is also legally binding. In this form you are agreeing to the
conditions of the form stating that the information you have given is true.
The last piece of information you are required to fill in this form is the date. Writing
the date is important as the person would know when we join the club. The date helps
the officials when they are calculating the amount you have to pay at the end of the
month.
In this lesson, we have seen that people are required to fill in forms in real life. We
have also noticed that most forms ask the same information: name, date of birth,
gender, address and contact number. We also learned that forms require you to give
correct information and are legally binding.
