PROJECT PROPOSAL

Dito Kabilang ka! The SK HBI Adolescent Health Hub


PROJECT TITLE

The Adolescent Health Hub shall be located in the Third Floor


PROJECT LOCATION Lobby of the Multi-Purpose Hall of the Barangay Hagdan Bato
Itaas, City of Mandaluyong.

INCLUSIVE DATES From April 2024 to December 2025

The SK HBI Adolescent Health Hub targets adolescents aged 10 to


19 years old, recognizing this critical stage of development where
individuals undergo significant physical, emotional, and social
changes. It aims to provide comprehensive health services and
support tailored to the unique needs and challenges faced by
adolescents, including reproductive health education, mental health
support, substance abuse prevention, and access to contraception

BENEFICIARIES/AUDIENCE
Additionally, the hub serves as a safe and inclusive space where
adolescents can seek confidential and non-judgmental assistance
from trained professionals, empowering them to make informed
decisions about their health and well-being. Through targeted
outreach efforts and community engagement, the Hub aims to reach
vulnerable and marginalized adolescents, including those from low-
income families, and marginalized sectors, ensuring equitable
access to essential health services for all young people.

• The establishment of the SK HBI Adolescent Health Hub stems from a


comprehensive assessment of the health needs and challenges faced by
adolescents within the community. Recognizing the critical importance of
adolescent health and well-being, particularly amidst evolving social and
environmental factors, the program was developed as a proactive
response to address these issues. The initiative is rooted in the principle
of youth empowerment, aiming to provide a dedicated space where
adolescents aged 10 to 19 years old can access a wide range of health
services and support tailored to their specific needs. The program draws
upon interdisciplinary expertise, collaborating with healthcare
professionals in the Barangay Health Center, educators, youth advocates,
and community leaders to ensure holistic and culturally sensitive care.

Through strategic partnerships with local healthcare facilities, educational
institutions, and youth organizations, the SK HBI Adolescent Health Hub
aims to leverage resources and networks to maximize its impact and
reach.

• Moreover, the program prioritizes community engagement and


participatory approaches, involving adolescents in the planning,
implementation, and evaluation of services to ensure relevance and
responsiveness to their needs. Ultimately, the establishment of the
Adolescent Health Hub reflects a commitment to promoting the health,
well-being, and empowerment of adolescents, laying the groundwork for
healthier and more resilient communities.

• The SK HBI Adolescent Health Hub offers a comprehensive range of


tailored health services and support aimed at meeting the specific needs
of adolescents aged 10 to 19 years old. These offerings include access to
reproductive health education, mental health support, substance abuse
prevention, and contraception and STI testing. Additionally, the hub

Through strategic partnerships with local healthcare facilities, educational
institutions, and youth organizations, the hub maximizes its impact and

• To offer a wide range of comprehensive health services tailored to the


specific needs of at least thirty (30) adolescents aged 10 to 19 years old
monthly. These services include reproductive health education, mental
health support, substance abuse prevention, contraception, and STI
testing, ensuring that adolescents have access to essential healthcare
resources.

• To foster youth empowerment by providing a safe and inclusive space


where adolescents can seek confidential assistance and support. By

prioritizing the autonomy and agency of adolescents, the program aims to
empower them to make informed decisions about their health and well-
being, ultimately promoting their overall autonomy and resilience.

• To promote community engagement and collaboration by fostering


partnerships with local healthcare facilities, educational institutions, and
youth organizations. Through strategic partnerships and collaborative
efforts, the hub aims to maximize its impact and reach, ensuring that
adolescents from diverse backgrounds have equitable access to essential
health services and support.

The approach taken by the SK HBI Adolescent Health Hub is vital for
addressing the health needs of adolescents within the community. By
conducting a comprehensive needs assessment, the hub gains insights
into the specific health challenges faced by adolescents, enabling the
delivery of tailored health services. These services, including
reproductive health education and mental health support, are designed
to address the identified needs effectively. Adopting a youth-centered
approach ensures that adolescents feel comfortable seeking assistance
and are empowered to make informed decisions about their health.
METHODOLOGY
Moreover, interdisciplinary collaboration among healthcare
professionals, educators, and community leaders ensures that
adolescents receive holistic and culturally sensitive care. Community
engagement plays a pivotal role in the program's success by involving
stakeholders in program development and implementation, ensuring that
the hub remains responsive to local needs. Regular monitoring and
evaluation allow for ongoing assessment of program effectiveness and
facilitate adjustments to optimize health outcomes for adolescents in the
community.

Adolescent Hub Committee- They shall be streamlining activity


proposals and strategies in linking the hub to different partnered
institutions to attain its sustainability.

Barangay Nutrition Council- They shall actively communicate with the


Specific persons to be SK Council in relying necessary information that is needed in the
involved and their roles improvement of the hub.

Barangay Council for the Protection of Childre- They shall provide


technical and administrative support to the Adolescent Hub of the SK
HBI.

• Reduction in Teenage Pregnancy Rates: One of the primary


indicators of success would be a noticeable decrease in teenage
pregnancy rates within the community served by the Adolescent
Hub. This can be measured through statistical data provided by
local health authorities or through self-reported data from
adolescents accessing the hub's services.

• Increased Contraceptive Use: Another indicator of success


would be an increase in the use of contraception among sexually
active adolescents. This can be measured through surveys or
interviews conducted with adolescents accessing the hub's
reproductive health services, as well as through sales or
distribution data of contraceptive methods provided by the hub.

Target • Improved Reproductive Health Knowledge: Success can also be

indicator reproductive health and contraception. This can be assessed
through pre- and post-test evaluations or surveys measuring
changes in knowledge levels among adolescents participating in
educational programs offered by the hub.

• Access to Sexual Health Services: The accessibility and


utilization of sexual health services provided by the Adolescent
Hub can serve as an indicator of success. This includes the
number of adolescents accessing services such as STI testing
and treatment, pregnancy testing, and counseling services
related to sexual health issues.

• Decrease in Risky Behaviors: Success can also be indicated by


a reduction in risky sexual behaviors among adolescents, such
as unprotected sex or early initiation of sexual activity. This can
be measured through self-reported behavior assessments
conducted as part of ongoing evaluations or surveys.

• Improvement in Mental Health Outcomes: Since mental health
issues often coexist with reproductive health challenges among
adolescents, success indicators should also encompass
improvements in mental health outcomes. This may include
reductions in symptoms of anxiety or depression, increased
access to mental health counseling services, and enhanced
coping skills among adolescents accessing the hub's services.

The SK HBI shall utilize feedback surveys and interviews to seek


immediate information for the improvement of the SK Adolescent
Evaluation Tools Hub. Data on the numbers of teens with Adolescent Health
Issues and Concerns shall also be referred to specifically the
pregnancy rates, HIV/STD positive rates, etc.
