Dear Dr.
I am writing this letter with regards to Mr. Hariom Gupta, 72 years male, who is presented to us
with the complaints of backache since 1 month.

INVESTIGATIONS:

• PSA (9.5.22): 106.36

• PSMA PET scan:


- Hetrogenous PSMA avid uptake in prostate parenchyma with extra prostatic
extension.
- No PSMA avid pelvic or RPLNS
- PSMA avid wide spread marrow based skeletal metastatic lesion.

• MRI Dorsal Spine (7.5.22):


- Multiple marrow infiltrating lesions in dorsal vertebra
- Compression collapse of D11 vertebrae
- S/O → metastatic lesions.

• TRUS Guided prostate biopsy: Acinae adenocarcinoma, G.S 4+4= 8

FINAL DIAGNOSIS: Adenocarcinoma Prostate.

ADVICE: At this stage I would like to recommend further management as follows:

1) To come to Mumbai at Sushrut Hospital, if possible.


2) To consider:
1. Inj. Elligard 22.5mg SC every 3 monthly.

2. Tab. Calutide 1 daily


3. Inj. Zoldonate 4 mg IV monthly.
4. Tab. Lycogem forte 1 daily
5. Tab. Calsmiq 1 daily
6. Abiraterone 250 mg 2 tab in the morning and two at night.
7. Wysolone 5 mg OD 1 daily.

Follow up Consultation done on 18.6.22


ADVICE:

1) To continue all medications as per instructions.


2) To come in second / third week of July 2022 with following investigations on 14th

3. Serum PSA
4. USG KUB
