This document provides a job hazard analysis for operating a forklift. It identifies several potential hazards associated with forklift operation including rollovers, crushing injuries, and equipment damage. For each job step, the document assesses the risk level and provides control measures to reduce the risk. The control measures include following manufacturer and site procedures, operating cautiously, not overloading the forklift, and parking in a safe area when finished. The goal of the analysis is to identify hazards and implement controls to safely complete the job.

The document outlines the hazards associated with operating a forklift and measures to reduce risk, such as following proper procedures, taking care, and thinking about safety.
The steps include operating only when trained, going at a safe speed, not overloading, following procedures, keeping a clear line of sight, not turning on inclines, no passengers, keeping loads stable and low, and not lifting loads near people.
Pre-operation checks include inspecting the forklift and completing checklists. Post-operation checks include completing further checks and maintenance tasks if authorized.

JOB HAZARD ANALYSIS FORKLIFT (to be used in conjunction with the JHA for each particular task) Note: This is just a guideline. Every JHA and Risk Analysis must be designed for each specific forklift and each specific work area and task. A JHA is a never ending document. There will always be a better way to get the job done.
