1.2 Purpose of WSH Incident, Investigation and Analysis Process

1.2.1 This document outlines procedure for the reporting of work-related

incidents, and to provide guidance on their investigation. It is also to

investigate the immediate and underlying causes of the incident and

ensuring remedial action is taken to prevent the incident from recurrence

1.3.1 This process is applicable to all incident/accident that happened in the

workplace involving employees, contractors, visitors etc

2.1.1 Workplace Safety and Health (WSH) Act 2006

3.0 Roles & Responsibility of the respective individuals carrying out
the incident investigation

3.1 HR Manager

 Responsibility to regularly assess the injured worker readiness to return

to work, and to ensure that they are paid the Statutory Sick Pay they

are entitled to.

 Responsible for ensuring that health and safety policies and procedure

are followed, and that an adequate report is written and filed.

3.2 Department Manager

 Responsible to form an investigation team

 Approve the PTW any and control the area in terms of incompatibilities

in work.

3.3 Department Supervisor

 Responsible for securing the incident scene to prevent further injury

 Completed Incident Investigation Report

3.4 WSHO

 To lead the incident investigation & come up with an investigation

report to the chairman of the safety committee

 Identify hazard such as risk & recommend control measure for the
work carry out through RA reviews

3.5 WSH Committee Member

 Inspect the factory after incident had happened

 Hold Meeting to discuss the findings & put up the require

recommendation to prevent incident from happening again

4.0 Establishing policy for protecting welfare of employee who report
incident

4.1 Introduction

4.1.1 Avi-Tech aim to be an incident free workplace & employees, clients,

vendors and other stakeholders are provided with an avenue to express

concerns on any serious WSH Act within the organisation.

4.1.2 In line with this commitment, the Board of Senior Management approved

and adopted this Whistle-blowing Policy to ensure that arrangements are in

place for whistle-blowers who may raise concerns about actual or possible

improprieties without fear of reprisals in any form.

4.1.3 Avi-Tech establish the Audit & Risk Committee also know as (“The A&R

Committee”) was found. It shall have the overall authority and oversight of

this Policy. The A&R Committee may in its absolute discretion delegate the

investigation of the whistle-blowing reports and implementation of this

Policy to such person as it deems fit.

4.1.4 It is the responsibility of all Board Members, officers, employees and

members of Avi-Tech to report any actual or possible improprieties.

4.2 Scope & Purpose of Whistle blowing policy

4.2.1 Establish a trusted avenue for members, employees, clients, vendors and

other stakeholders to report serious WSH wrongdoings or concerns without

fear of reprisals when whistleblowing in good faith.

4.2.2 Ensure arrangements are in place to facilitate independent investigation of

the reported concern and that appropriate follow-up actions will be taken.

4.2.3 Encourage members and employees to raise concerns at an early stage to

an internal authority so that actions can be taken immediately to resolve

them.

4.3 Protection against reprisal and confidentality

4.3.1 Harassment or Retaliation

4.3.1.1 Avi-Tech recognizes that the decision to report a concern can be

difficult, not least because of the fear of reprisal from those responsible

for the improprieties. It will not tolerate harassment or victimization and

will take reasonable and appropriate action to protect whistle-blowers

when they raise concerns in good faith

4.3.2 Confidentiality

4.3.2.1 Avi-Tech will do its best to protect a whistle-blower’s identity when the

whistle-blower indicates that he or she does not want his or her identity

to be disclosed.

4.3.3 Anonymous Report

4.3.3.1 Avi-Tech is committed to respect the rights of its employees/whistle-

blowers, which includes the right of an accused person, if absolutely

necessary, to know the identity of his or her accuser. The identity of the

whistle-blower would not be disclosed unless it is absolutely necessary

for the purpose of the investigation and/or subsequent action, and never

disclosed without prior discussion with the whistle-blower making the

report.
