the online Self-monitoring Report (SMR)
Workshop.
1. That the proponent shall establish a sampling point on effluent
discharges for sampling and monitoring purposes;

2. That all permits and clearances shall be regularly secured from the
concerned national and local offices during the project implementation;

3. That a quarterly Self-Monitoring Report (SMR) shall be submitted to this


Office for validation within fifteen (15) calendar days after the end of each
quarter and a Compliance Monitoring Report (CMR) shall be submitted
semi-annually as an attachment of the second and fourth quarter SMRs
constituting as Module 5;

4. That in case of abandonment, the proponent shall notify this Office within
three (3) months prior to the abandonment and the proponent shall
submit its abandonment mitigation plan;

5. That the proponent shall conduct continuously an Information


Education Communication (IEC) Campaign to the surrounding
community to inform them of the environmental impacts and
corresponding mitigating/enhancing measures during the
implementation of the proposed project;

6. The proponent shall employ a Pollution Control Officer (PCO) in


accordance with RA 8749 and RA 9275 particularly DAO 26, S. of 1991,
who shall handle the environmental aspects of the project, which shall
have the following responsibilities:

7. That a 2’ X 4’ billboard containing this message: “Notice to the Public,


This ________________________________________ has been issued an
Environmental Compliance Certificate (ECC-OL-R13-2021-XXXX) by
the Environmental Management Bureau of the Department of
Environment and Natural Resources, Caraga Region, on March 17,
2020.” This message must be installed at entry and exit points of the
project facing the road to inform the general public within thirty (30)
days from receipt of the certificate. A picture of compliance to the posting
of the billboards shall be submitted to this Office within ninety (90) days
from receipt of the ECC;

8. That the EMB-13 reserves the right to cancel or withdraw this certificate
for any misrepresentation of facts vital to the issuance of this certificate
and for any violation of the conditions stipulated hereof;

9. No other activities should be undertaken other than what has been


stipulated in the submitted IEE Checklist. Should there be an expansion
of the project beyond the project description, construction of other
structures beyond those stated in the IEE Checklist or any change in the
activity shall be made subject to a new Environmental Impact
Assessment;

10. In case of transfer of ownership of this project, these same conditions


and restrictions shall apply and the transferee shall be required to notify
the EMB Regional Office concerned within fifteen (15) days as regards to
the transfer of ownership;
