Occupation- Farmer

Socioeconomic Status- Poor economic condition

Chief Complaints:-

Patient has as such no complaints in his RE.He has come for routine check-up.

History of present illness:-

Patient was operated in his RE 3 months back in Malda Medical College and Hospitial.He was operated
as he developed gradual onset of dimness of vision in his both eye for last 10 months and the problem
was more in RE.He consulted eye surgeon in this hospital ; Who after examination told that he has
cataract in his both eye and decided to operate RE first. According to the the patient he has undergone
cataract surgery along with intraocular lens implantation in his RE.2-3 days after operation he was able
to see everything in distance with his RE but there was difficulty during near work. one and half month
after operation doctor prescribed him glass and he uses glass only during near work.At present he has
come for routine check up only.
History of Past Ocular illness:-

Nothing Significant

History Of Systemic illness:-

Patient has no history of diabetes,hypertension, thyroid disorder,tuberculosis or any other systemic


disorder and he never had any type of operation.

Family History:-
Nothing Significant

Personal History:-

He is a smoker but not addicted to alcohol.

No history of any allergic disorder

General Physical and Systemic Examination:-

Reveals no abnormality.

Ocular Examination:-
Visual Acuity:- RE 6/6p without glass and 6/6 with his own glass. RE near-N6 with own glass. PR acc in all
quadrants.

EYE LID and Eye lashes:-Normal in position and is no entropion,ectropion,blepharitis or trichisis.

Conjunctiva:-No congestion. Scar mark seen near limbus ,superiorly.

Sclera:- No congestion.Scar mark seen superiorly near limbus, from 10-2 oclock position

Cornea:- scar mark seen from 10-2 oclock position.Rest of the cornea is clear.

Anterior chamber depth:- Normal

Iris:- iridodonesis present.One small atrophic patch seen over the iris in 9 oclock position.

Pupil:- Round,regular, reacts briskly.Shiny glassy reflex found behind the iris plane.

purkinje’s images:- All the 4 purkinje’s images are present.

Digital Tonometry:- Normal

Extraocular muscle movement:- Normal

Lacrimal Apparatus:- Appears to be normal


General Examination:- Grossly examined ( take Bp, pulse, Temparature, Respiratory rate, Anaemia,
cyanosis, Jaundice , clubbing )

Case summary :- yyy, 65yrs male patient,who is a smoker, undergone cataract surgery with intraocular
lens implantation I his RE 3months back.On examination, his visual acuity is ….with out glass and with
glass the visual acuity is…..and with pin hole it is…..the Scar mark found in the conjunctiva,superiorly
and also in the cornea.Iridodonesis present and there is atrophic patch in the iris.Shiny reflex found
behind the iris plane and all the purkinje’s images present.

So from the above points,my provisional diagnosis is RE Pseudophakia.


(N.B;- irididonesis present in pseudophakia is very mild and may not be seen with torch light. So , some
examiners may pull your legs by saying that how have you noticed it with torch light. So if you do not
utter this….no hram, and also about atrophic patch….do not utter it ,if it is not seen)

N.B:- ……mention the visual acuity according to your case. If the patient has 6/6 vision with glass
….further pin hole vision need not to be written….Hope you understood.

PSEUDOPHAKIA CASE-2

Chief Complaints:-With out glass RE distance visual acuity is defective but there is no difficulty during
near work after operation

History of present illness:-

Patient was operated in his RE 3 months back in Malda Medical College and Hospitial.He was operated
as he developed gradual onset of dimness of vision in his both eye for last 10 months and the problem
was more in RE.He consulted eye surgeon in this hospital ; Who after examination told that he has
cataract in his both eye and decided to operate RE first. According to the the patient he has undergone
cataract surgery along with intraocular lens implantation in his RE.After operation patient was suffering
from defective vision for distance in his RE but he had no problem during near work. one and half
month after operation doctor prescribed him glass and with glass he is able to see both distance and
near objects clearly.

Visual Acuity:- RE 6/60 without glass. 6/6 with his own glass and with pin hole also. . RE near-N6 with
his own glass and N8 without glass. PR acc in all quadrants.

Pseudophakia Case:-3
Chief Complaints:-With out glass RE distance visual acuity as wel as near vision is defective after
operation
History of present illness:-

was more in RE.He consulted eye surgeon in this hospital ; Who after examination told that he has
cataract in his both eye and decided to operate RE first. According to the the patient he has undergone
cataract surgery along with intraocular lens implantation in his RE.After operation patient was suffering
from defective vision for distance as well as for near in his RE . one and half month after operation
doctor prescribed him glass and with glass he is able to see both distance and near objects clearly.

Visual Acuity:- RE 6/60 without glass. 6/6 with his own glass and with pin hole also. . RE near-N6 with
his own glass and N18 without glass. PR acc in all quadrants.

Pseudophakia Case:-4
Chief Complaints:- Gradual dimness of vision in his right eye for last 5 months.

History of present illness:- Patient had undergone cataract surgery with intraocular lens implantation in
his RE 3 years back in Malda Medical College and Hospitial. After operation patient was doing well with
his glass and had no problem . It is for the last 5 months that the patient is feeling gradual dimness of
vision in his right eye and in his version he is feeling that he has again developed cataract in his right eye.

Visual Acuity:- Right eye visual acuity is 6/36 with out glass( PR acc in all quadrants) and this vision is
not further improving with pin hole. Near vision is N24.

purkinje’s images:- All the 4 purkinje’s images are present , but 4th image is distorted.

months the patient is feeling gradual dimness of vision in his right eye and feeling that he has again
developed cataract.On examination, his visual acuity is ….with out glass and with glass the visual acuity

So, from the above points my provisional diagnosis is Pseudophakia with posterior capsular opacity in
his right eye.
