PM Transition Template

Purpose
The purpose of transition planning is to layout the tasks and activities that need to take place to
efficiently transfer an Corporate IT PMO project to another PM.

In preparation for the departure of key staff within the organization, this planning template assists in
the effort to lessen the impact of those departures by capturing their knowledge.

The PM Transition Template is a knowledge transfer tool, that will:

 Ease role transition efforts


 Capture essential position and project information
 Provide a process to transfer knowledge in a scheduled, structured manner

(The transition plan is used in conjunction with the Project Charter, Business Requirements, and
Technical Design documents and is not intended to repeat information already found in those
documents. Any changes to those documents should be made and recorded accordingly.)

recurring meetings and responsibilities.

potential litigation or negative public scrutiny.

The purpose of this component is to provide an understanding of the active issues/risks inherent with
