    PETITIONER, ____________________, through himself and unto


    this Honorable Court most respectfully states the following:

    1. That he is above twenty-one (21) years of age, having been born on


    ____________________, and is a Filipino Citizen;

    2. He is a resident of ____________________ and has been a resident


    thereof since the year 2013, with Office Address at
    ____________________, and with contact no. ____________________;

    3. Petitioner passed the ______ Bar Examinations and was admitted to


    the Philippine Bar on ____________________, with Roll No.__________,
    as shown by his Certificate of Membership in the Philippine Bar, attached as
    ANNEX “A” and made an integral part of this Petition;

    4. Petitioner is a member of the Philippine Bar in good standing, as


    shown by his clearances from the Office of the Bar Confidant of the
    Supreme Court and from the Integrated Bar of the Philippines (IBP)
    National Office, respectively indicated, attached, and made an integral part
    of this Petition as ANNEX “B” and ANNEX “C;”

    5. He is likewise considered to be in good standing by the IBP


    ____________________ Chapter, as shown by the certifications issued by
    two (2) executive officers of the said IBP local chapter— the Chapter
    President and Chapter Treasurer, respectively indicated and attached as

    ANNEX “D,” and ANNEX “E,” and likewise made an integral part of this
    Petition;

    6. Petitioner has not been convicted in the first instance of any crime
    involving moral turpitude;

    7. He had fully paid for his IBP membership dues for the year _____ as
    shown in his Certification from the IBP National Office and in his proof of
    payment in the form of an Official Receipt with No. _________,
    respectively attached as ANNEX “F” and ANNEX “G,” and both made an
    integral part of this Petition;

    8. Petitioner is currently not employed in government service;

    9. That, pursuant to the requirements set by the Honorable Court under


    existing rules, he attaches the following annexes to be made integral parts of
    this Petition for Notarial Commission:

     Location Map of Notarial Office, attached and made part of


    this Petition as ANNEX “H;”

     Three (3) passport-sized and unretouched photographs of the


    Petitioner, taken within thirty (30) days prior to the filing of
    this Petition, with his signatures at the bottom part of the
    photographs, as ANNEX “I;”

     Impressions of his Dry Seal to be used in his notarial acts, as


    ANNEX “J;”

     Specimen signatures to be used by Petitioner in his notarial


    acts, as ANNEX “K;” and

     Petitioner’s Professional Tax Receipt for the current year,


    issued by the City of _________, with No. ________, as
    ANNEX “L;”

    10. That Petitioner has not previously applied for a notarial commission in
    any other commissioning court.

    this most Honorable Court that this Petition for New Commission as Notary
    Public by the undersigned be granted.

    Other reliefs deemed just and equitable by this Honorable Court are
