I. PURPOSE

This method of statement describes the procedure of site grading works at Silay Solar Farm 2A and 2B at
Barangay Eustaquio Lopez, Silay, Bacolod. The site grading works involve the cut and fill as well as the
compaction and levelling to a desired elevation. The maximum grading limit proposed for the plant is 12% which
adheres to the standard practice being applied to solar farm developments, the objective of which are to
establish cost efficiency and design optimization.

IV. EXECUTION

001. Survey and Setting-out

1) Use RTK and total station to confirm the cut and fill area and elevation; compare the topographic
survey plan (provided by client) with actual layout & elevation. If there is a discrepancy between the
plan and actual, contractor to inform the client for adjustments.
2) For every 25 meters a stake point will be driven as control point for excavation and back filling.
3) During the entire activity, stake points will strictly follow to eliminate over cutting and over filling. Every
layer for back filling must be controlled at 300mm.

002. Clearing and Grubbing

Use bulldozer to clean and remove all deleterious materials and put them in one place. The backhoe will be
used to load the waste materials onto the dump truck. The Dump truck will haul out and dispose the waste
materials to designated area by Client.

003. Excavation and Hauling

Guided by the stakes, excavate the area to be cut until it reaches the design elevation indicated in the approved
drawing. The cut sand will be used as fill on low elevation areas. Strategic soil transfer will be observed to
minimize the transport distance.

004. Backfilling and Compaction

1) After dumping the sand on the fill area, the bulldozer will be used to spread the sand. Fill height will be
strictly maintained at 300mm
2) A roller compactor will be used to compact the fill sand and make 5-6 passes.
3) A field density test will be conducted every layer of compaction.

V. HEALTH AND SAFETY

Hazard analysis with risk assessment shall be carried on the project site. Hazards in a particular activity and the
control measures shall be briefed to the workers during safety induction and toolbox meeting.

005. Safety Equipment

Minimum Personal Protective Equipment (PPE) required by MCC-Citicore renewable Energy Corporation and
NAVI GEO-CONSTRUCTION CORP. procedure such as, but are not limited to hard hat, safety shoes / boots,
vest and face mask will be issued to all workers involved in the activity along with other required PPEs such as
face shield (if needed), hand gloves and ear plugs (if needed). Reflectorized vest with shall also be issued to
workers to ensure greater visibility especially when working at night. Other special PPE may be prescribed as
per risk assessment.

006. Health and Safety General Requirements

1) All HSE regulatory, contractual and project requirements will be adhered to at all times during the
performance of the activity.
2) Consider safe work load limit in equipment and operating parameters when using unit to lift and unload
materials.
3) A Work Permit (PTW) must be secured before the start of any activity.

007. Permit to Work System

For a number of work activities, a Permit to Work system may be used. The Permit to Work system is based on
a written procedure including suitable form(s) used to control potentially dangerous operations by authorizing
personnel to carry out specific activities within a certain time frame, and at the same time identifying the main
precautions needed to complete the task safely.
Activities that will require a Permit to Work include, but are not limited to:

1) Excavations/trenching (more than 300mm in depth, with additional controls where work in the vicinity of
services.
2) Hot works (welding, grinding, cutting, or danger of an ignition source)
3) Electrical works
4) Heavy Lift/Vehicles (heavy vehicle transport, off road, non-4x4 access)
5) Area access (where Contractors take control of a designated area)
6) Other work or locations designated by Employer or NAVI GEO-CONSTRUCTION.

008. Health and Welfare Arrangement

1) A first aider should be available at the work site to provide first aide just in case of minor injury occurs.
2) Adequate first aid kit will be made available at the work site with medicines.
3) Referral to the nearest partner hospital for work related medical emergencies.
4) Resting area and mess hall area will be designated.
5) Adequate supply of potable water will be made available for workers.

009. Fire Precautions

1) Smoking is not allowed on site.


2) Fuel supply shall be stored at the temporary facility area and authorized personnel shall be appointed
for this item.

010. HSE Information to Personnel

1) All workers involved in the activity must go through the proper safety orientation on how to conduct the
activity safely prior to start of work.
2) Based on this method statement, a Job Safety Analysis (JSA) shall be prepared for the activity and
discussed with workers for understanding and compliance.
3) Toolbox talk shall be conducted by the Supervisor prior to start of each work shift with the assistance of
the site Safety personnel.

011. Job Safety Analysis (JSA)
This section formulates the control of hazards in the workplace and shall be done before start of any activity
on site. This is normally prepared by the supervisor and disseminate to his workers afterwards in order to
be aware on the hazards presence or exist at the workplace. To mitigate or remove hazards in the
workplace, Elimination, Reduction, Isolation and Personal Protective Equipment must be use as alleviation.

VI. HEALTH AND SAFETY

012. Purpose
To be able to inform the employee on what to do in case of emergency. This Procedure applies to NAVI GEO-
CONSTRUCTION personnel and all the contractors working within the Project in preparation and in responsive
action in case of emergency.

013. Responsibility

Managers and Supervisors – shall be responsible for dissemination and implementation of this Procedure to all
employees working in the Project and see to it that it shall be reviewed regularly to check applicability and
revise if necessary.

HSE Officer – Shall be responsible in facilitating and scheduling of emergency drills.

Security Personnel - Assist the emergency response team during emergency rescue, cordoned off the area and
controls the crowd. They shall be responsible for the Starting or assisting in the evacuation of the site; check
using that the site is empty and everyone has left.

014. Procedure
In case of fire

1) Raise the alarm immediately upon discovery. Activate fire alarm system (if already available).
2) Immediately inform Managers, Supervisors or HSE Officer through emergency contact numbers (To be
posted on HSE Bulletin Board and other Information board all throughout the site).
3) Attack the fire if you are trained to do so using proper fire extinguisher.
4) If the fire is too large evacuate the area and follow the Emergency Evacuation Plan and proceed to the
identified assembly area.
5) Follow the instructions given by the fire marshals.
6) Headcount must be conducted to ensure that no one is missing or trapped.
7) Do not leave the assembly area, wait for further instructions.
8) Return only to your work areas if you are told to do so.

In case of Earth quake.

If you are at Working Area:

1) Don’t Panic. Do not run.


2) Stay away from any tall structures that may be possibly damage such as overhead power lines etc.
3) When the shaking stops – proceed to the assembly area as instructed by the emergency
4) response team.
5) Do not leave the assembly area. Wait for further instructions.

Medical Emergency Procedure

Upon discovery immediately call medical emergency phone number. Provide the following information:

1) Nature of medical emergency


2) Location of the emergency
3) Name and phone number from which you are calling.
4) Do not move victim unless absolutely necessary.
5) Call the First Aider to provide the required assistance prior to the arrival of the professional medical
help:

 If First Aider are not available, as a minimum, attempt to provide the following assistance:

1) Stop the bleeding with firm pressure on the wounds (note: avoid contact with blood or other bodily
fluids).
2) Clear the air passages using the Heimlich maneuver in case of choking.
3) In case of rendering assistance to personnel exposed to hazardous materials, consult the Safety Data
Sheet (SDS) and wear the appropriate personal protective equipment. Attempt first aid ONLY if trained
and qualified.
4) Bring the victim to the nearest hospital if necessary and if he/she needs an immediate medication.

015. Emergency Response Team

An emergency response team to handle emergencies relating to traffic accident, fire, medical emergencies, and
other emergencies based on identified shall be organized to cover the work site.
Members of emergency response team shall undergo standard First Aid Training from the Philippine Red Cross
or in any accredited training center.
The Emergency response team shall facilitate emergency drills once semi-annually headed by HSE officer.

Hazard Classification
