COMPANY CODE OF CONDUCT ACKNOWLEDGEMENT FORM

By signing here below, I hereby acknowledge that I have completely read and fully understand the
Customer Frontline Solutions Company Code of Conduct.

I shall not, at any time or in any manner, either directly or indirectly, divulge, disclose or communicate
to any person, firm, corporation or other entity in any manner any confidential information I obtained
while engaged by the Company. In this regard, I ought to reasonably understand such information to be
confidential.

Failure on my part to strictly observe the stipulations and conditions in the Agreement shall oblige me,
the offending party, to pay liquidated damages in the amount of not less that PHP 10,000,000,00

This Agreement shall be binding upon and inure to the benefit of the parties, their successors, assigns
and personal representatives.
