This memorandum summarizes a final investigation report into a structural fire at a nipa hut in Brgy. Bio, Tagudin, Ilocos Sur on December 31, 2022. FACTS OF THE CASE:

A. This pertains to a structural fire that transpired at about 31 1210H December


2022 , MDRRMO Engr. Michael Padayao (Contorl 6) reported to this station through
radio that a fire broke out at Brgy. Bio, Tagudin, Ilocos Sur. Pen 1 firetruck driven by
SFO1 Julius Derik M Opinaldo together with INSP NILO B OCAMPO (ground
commander), FO3 Michael R Malasig and FO3 Nikki Christian C Bondad immediately
proceeded to the fire scene followed by kia firetruck driven by FO1 Ronel O Racuya
together with FO1 Mark Hyro C Peralta and FO1 Cris Jhon P Lagazo. Upon arrival at the
fire scene, the fire was already under control with the help of their neighbors performing
bucket relay. Immediately crew of Pen 1 conducted firefighting. Mopping operation was
also conducted to prevent the fire from rekindling that lasted for five (5) minutes and
declared fire out by INSP NILO B Ocampo (Ground Commander) on or about 1220H.

B. Initial investigation conducted revealed that the burned structure was a nipa hut
made of light materials,cogon grass, bamboo and wood and owed by the Longoria
Family located at the back portion of the house. No casualties nor injuries reported
during the fire incident.

C. Based on the submitted affidavit of loss, the estimated cost of damages is


Php 6,200.00 more or less

DISCUSSION:

Based on the submitted sworn statement of the victim, Mr. Jimmy G Logoria, that
morning around 1140H, the whole family proceeded to Brgy. Cabaroan,Tagudin, Ilocos
Sur to attend a family gathering. Upon arrival at the venue, a named Jannella G
Lambinicio, thier nieghbor chat her daugther, Jamie Rose L Longoria, informing them
that their was a fire broke out of their house. After knowing the news, the whole family
immediately went back home to verify the news. Upon arrival, they found out that the
nipa hut located at the back of their house was already burned and also slightly damage
the window and door on the second floor of the house. The victim believe that the
possible cause of the fire was a fire cracker (kwitis) that accidentally fell and ignite the
cogon grass roofing of the nipa hut that cause the burning of the structure. The victim
also stated in his affidavit of non complaint that he is not blaming anyone about the fire
incident.

Based on the submitted sworn statement of one of the witness, Ms. Janella G.
Lambinicio, on that morning, she was washing the dishes when she hear some blast, in
her knowledge it was a homemade pvc cannon (Buga). After washing the dishes they
notice together with her grandma a very thick smoke coming from their neighbor, she
immediately chat Jaime Longoria, daughter of the victim that there is fire in their house.
Afterward,she shout for help to the neighborhood to help in the suppression of the fire.

Based also to the statement of some of their neighbor who first respond to their
house, that morning they notice some blast of firecracker around the barangay. As all
preparing for the celebration of new year when suddenly someone shouted for help that
their is fire. Immediately they responded and found out that something is burning at the
back portion of the house of the Longoria Family, they tried to extinguished the fire by
means of bucketing until the arrival of the Bureau of Fire protection. They also stated that
they had no choice but to enforce forcible entry in order to enter the house to have a
much faster suppression of the fire.

The origin of fire was caused by firecracker that accidentally fell and ignite the
cogon grass roofing resulted to the burning of the structure and slightly damage the main
house of the victim.

FINDINGS:

Based on the investigation conducted, taking all considerations all the facts and
attending circumstances, under the rule applicable, the cause of fire is accidental in
nature due to firecracker as no evidence would show that it is intentional and deliberately
set.

RECOMMEDATION:

In the light of the foregoing, it is respectfully submitted that this case be


considered close without prejudice to re-investigation if there shall be subsequently
complaint arising and that ,witnesses or other evidence be found which shall warrant
the re-opening of the case.
