Daily Process Recording

Activity: HOME VISITATION

Name: LOUELLA
Time Frame of Activity: December 4, 2023 (8:00AM-11:35AM)
Venue of Activity: Brgy Carpenter Hell , Koronadal City, South Cotabato
Participants Involved: Social work Intern, Client

Objectives:

 To determine the needs/problems of the client and provide some appropriate action.
 To make known of the client’s bio, psychosocial, spiritual needs through the constructed
classifications of her status or life situations.

 Presented Problem:
The client expressed that her relationship with her family is not going in good situation due to
the repetitive abuse of her husband which affected their children. In this situation they loss
their respect and interest to her because they describe their situation as negative influence. It
troubled her inside to the point that she sought help to the agency.

 Client-Worker Interactions: (Describe how did the client respond to the

interventions that you provided?)
The client was clearly reserved during the home visitation, but as the interview started and
progresses, the social work intern establishes a rapport with her through words of
encouragement and openness. When that happened, she tends to open up about her feelings.
The majority of the time, the client is attempting to recall every detail that she was required to
provide in response to the questions posed, and the fact that the social work intern is not
pressuring her to finish makes it easier for her to think through her responses. As a result, the
client started to smile and speak more when I asked her questions; she started to open up.
Without hesitation, she even disclosed about her family history, financial situation, and other
relevant details.
 Student’s feeling and Observation:
The social work intern found it daunting to witness her client's lack of response over time, as it
allowed her to assess her abilities and experiences in managing similar cases or clients. Based
on her observations, she concluded that the client exhibited the potential for self-help in solving
her problems with the assistance of the social work intern and that her background indicated
that she possessed strengths that could be utilized for future interventions on her behalf. The
social work intern has faith in the client to overcome all obstacles, even in the face of a
challenging situation.

 Learning’s and Insights:

This interview taught me how important it is to develop "trust" with your clients because it's the
first step toward starting a helping relationship because it creates a channel between them and
the social work intern or worker. One of the fundamental principles of social work is worker self-
awareness, which should be upheld at all times. This is because it can prevent
miscommunications and disputes that may arise from acting or offering advice that is
inappropriate in the particular circumstance. A prompt assessment of the client's condition or
awareness of it is essential to the practice of social work.

 Plans for the Next Meeting:

After reviewing the information gathered, the social work intern will follow up or manifer with
the client to inquire about any additional support needed for her issues and concerns. They will
also schedule a date and a time to discuss potential resources and referral systems if necessary.
