AFFIDAVIT OF LATE FILING


I, 2LT. RAMON ALTAMIRA SARMIENTO PAF (ret), Filipino citizen, of
legal age, married, and presently residing at F11 8 th-19th St., Barangay 183, Villamor Air
Base, Pasay City, after duly sworn to in accordance with law, depose and say:

That I am a Pensioner of AFPPGMC having served the Armed Forces of


Philippines-Philippine Air Force from December 29, 1976 to May 31, 2002 pursuant to
Military Retirement Special Order No. 215 Paragraph 5 dated December 10, 2001 (a
copy of which is hereto attached);

That there was a delay in filing/updating my monthly pension with AFPPGMC


because I am not aware and knowledgeable of the procedures and filing date thus, my
monthly pension was cut off since 2021;

That when I visited my daughter in California, United States of America (USA)


who is presently working and residing there, I never become an American Citizen neither
applied for US Citizenship, I am a Filipino Citizen as evidence by my Philippine Passport
(copy of which is here attached) and to restore my monthly pension effective upon cut
off;

That I executed this affidavit to inform the proper authority of the truth of the
foregoing facts for whatever other legal purpose it may serve.

Parañaque City, Metro Manila, Philippines.
