I. PURPOSE
Pursuant to Republic Act (RA) 9155 or the “Governance of Basic Education Act of 2001”
and RA 10533 or the Enhanced Basic Education Act of 2013 Department of Education implements
programs, projects and activities to ensure access to and improve the quality of Basic Education.
Relative to this, Department of Education issued DO 029 s 2022 dated June 22, 2022 entitled
“Adoption of the Basic Education Monitoring and Evaluation Framework to guide DepEd operating
units across governance levels in the conduct of M&E” aligned with the Department’s Basic
Education Development Plan (BEDP) 2030.To ensure the effective and continuous implementation
of these Programs and Projects, School Monitoring and Evaluation Plan will be crafted to help
track and assess the results, monitor the progress of the implementation and minimize the risk of
Programs/Projects failure. It is a living document that should be referred to and updated on a
regular basis.
III. SCOPE
To ensure the effective implementation of School M&E Plan, quarterly monitoring and
evaluation will be conducted by each monitoring team in accordance with the school existing
terms and policies. Reporting of results will be followed during SLACs, SMEPA and PIR
(Performance Implementation Review), distribution of SRC (School Report Card), posting on the
Transparency Board and others. Different stakeholders will be involved in the conduct of the M&E
activities specifically the members of SPT (School Planning Team) and SGC (School Governance
Council). Funds and other expenses will be charged to School MOOE and other School existing
funds.
The School head will lead the implementation of the M&E Plan together with the Project
Teams. Different group meetings, Focused discussions and brainstorming will be conducted for
the Planning Stage. Feedbacks will be considered and analysed to arrive in one comprehensive
plan. Suggestions and opinions of other stakeholders will be gathered thru different online
platforms, surveys and on-site interviews.
In Data gathering, there is a specific team who will consolidate. They will present the results
using graphs, diagrams and others. They shall provide feedbacks for continuous improvement of
the Plan.

V. TERMS OF REFERENCE
The Principal shall be the over-all leader of the School M&E System. She is the one who
develop together with the School M&E Team the School M&E Plan. She has the authority,
accountability and responsibility to ensure that the information and results from the M&E
activities are accurate and can be used in the development of programs, projects and activities.
Decisions and directions on school issues and other matters arising from various M&E activities
are on her hands. She will communicate school concerns to the school division office during
meetings or conferences, oversee the conduct of M&E activities, engage different stakeholders in
the implementation of the activities, prepare M&E Reports and conduct quarterly Program
Implementation Review (PIR)to track the development and achievements of the projects and to
make necessary adjustments to minimize the failure.
Other school personnel and stakeholders as part of the School M&E Team shall perform
their duties and responsibilities such as conducting M&E activities in accordance with M&E
standards. They can discuss, provide feedback, insights, lessons and other issues gathered during
M&E activities among the team, School Head, fellow teachers and other stakeholders. They can
participate in school M&E Reporting activities in SMEPA/Dsmepa, SLACs and PIR that serve also
as the venue for Results appreciation and evaluation. Team members can also apply results in
improving teaching-learning strategies and individual performance.
To ensure collaboration among the team members, regular meetings will be conducted.
Minutes of the meetings and other records will be consolidated and properly filed for future
perusal.
To gather the results and data the school will use the existing M&E Tools. We also use our
localized tools such as checklist, survey and interviews thru different platforms. To analyse the
gathered data we will presented them using graphs.

The school head together with the team leaders and the school Administrative Officer 1 will
consolidated the report to be filed in the SBM Corner, displayed in conspicuous areas of the
school, and included in the SRC. Data and results will also be presented during the State of the
School Address and during meetings with other stakeholders. These will also serve as a guide for
the continuous improvement of the projects or for finding the appropriate solutions if there are
some problems in the implementation and gathering of results.
