STATEMENTS of WORK
Roll Up Doors Visual Warnings
1. General information:
1.1. This document is the Statements of Work for the supply and installation of roll up doors visual warnings.

1.2. Project: Installation of Roll Up doors visual warnings

1.3. Address: Lots 1-10, Block 4, Phase 3, Lima Tech Center, Special Economic Zone, Municipality of Malvar, Batangas
City, 4233 Philippines

1.4. Roll up doors visual warnings will be applied to all roll up door in both internal and external leaf warehouse.

2. Scope of work

The contractor needs to supply and install visual warnings coordinated to the roll up door settings.

2.1 Visual Warning devices

 Light indicator/ strobe light

3. Designing and works execution requirements

3.1 The work site layout with the location of the equipment is given in Appendix No. 2.
3.2 The Contractor must appoint an English-speaking Site manager to coordinate the work performed with the Customer
and representatives of the equipment manufacturer.
3.3 The Contractor must appoint English-speaking Safety Officers to supervise the works in progress.
3.4 The scope of work includes also:
 Supply and install visual warning devices
 Conduct wiring connections
 Laying of wires
 Commission and testing of devices
3.5 Prior to the start of work, the Contractor must develop Work Method Statement and submit it for Customer
approval. The Contractor will be allowed to work only after the approval of the Work Method Statement and
familiarization of all the Contractor's employees with it.
3.6 Work at heights activity must have Work at Height’s Permit.
3.7 The Contractor must ensure that the electrical works is performed by the Contractor's employee who has a proven
qualification and experience in electrical activities.
3.8 Tools and equipment for electrical work shall be provided by the Contractor.
3.9 Works are carried out during the working hours of the Customer. If it is necessary to work at other times, the work
schedule should be agreed by the Parties additionally.
3.10 The design layout, device’s specification and gantt chart for the project must be submitted to customer prior the
start of the project.
3.11 The Contractor provides the transportation (shuttles) of the Contractor's personnel to the Customer's site.
3.12 Contractor must provide a detailed schedule for the designing and works execution.
3.13 Since the access of third parties to the Customer’s enterprise is limited, the Contractor informs the Customer
about the number of the Contractor’s employees who must be provided with access to the factory.

4. General Requirements
4.1 Responsibilities distribution between Customer and Contractor is given in Appendix No. 1
4.2 Works under this STATEMENTS of WORK must be carried out in accordance with the requirements of applicable laws,
standards and regulations of the Republic of the Philippines and JTI in the field industrial safety, fire safety and
environmental protection, including, but not limited to:
1. JTI AMC Contractor EHS Policy
2. Labor Code of the Philippines
3. Occupational Safety and Health Standards of the Philippines
4. Labor Advisory No. 04–19 Guide for Compliance of Establishments to DO 198-19
5. Philippine Electrical Code
6. The National Building Code of the Philippines
7. Necessary Local licenses, certificates and work permits.
4.3 Lifting equipment must have necessary inspection reports, certificates and other documents required by law.
Personnel must have all the necessary permits to perform work. The place for charging the lifting equipment is
agreed with the Customer.
4.4 All employees involved in the performance of the Works must have all necessary training, permits, certificates in
accordance with the requirements of the Republic of the Philippines legislation.
