MOLECULAR CHARACTERIZATION OF MULTI DRUG RESISTANT KLEBSIELLA


PNEUMONIAE IN HOSPITALIZED PATIENTS
Introduction :

Klebsiella pneumoniae, belonging to the Enterobacteriaceae family, is a natural inhabitant of the


gastrointestinal tract microbiome of healthy human and animals. It is a common opportunistic
hospital-associated pathogen, accounting for about one third of all Gram-negative infections
overall. It is involved in extra-intestinal infections including urinary tract infections, cystitis,
pneumoniae, surgical wound infections and life-threatening infections, such as endocarditis and
septicemia. (Haryani, Noorzaleha et al. 2007)
Due to the high rates of resistance to antibiotics, K. pneumoniae was considered as one of the most
frequent agents of infectious diseases and significant menace to public health . The resistance to
antibiotics is a complex multifactorial process. Nevertheless, it reflects evolution in action, related
to the continuous exposure to antibiotics.(Navon-Venezia, Kondratyeva et al. 2017) Consequently,
the selective pressure gives rise to the development of many genetic mechanisms . The acquired
resistance over the years has led to the emergence of multidrug-resistant (MDR) and extensively
drug-resistant (XDR) strains that exhibit resistance to nearly all available antibiotics, without
possible treatment options.(Falagas and Karageorgopoulos 2008) The resistance of K. pneumoniae
to many antibiotics is due to multiple mechanisms such as modification of antibiotic target sites,
alteration of metabolic pathways, activation of efflux pump systems, change in the permeability of
membrane and release of antibiotic-inactivating enzymes.(Lambert 2005)
Klebsiella pneumoniae has been associated with different types of infections and one of the most
important aspects of Klebsiella is the emergence of multi-drug resistant strains particularly those
involved in nosocomial diseases. Klebsiella pneumoniae plays a major role in the worldwide
burden of antibiotic resistance. (Blot, Depuydt et al. 2007)
Problem statement:
Klebsiella pneumoniae plays a major role in the worldwide burden of antibiotic resistance. In this era of
antibiotic resistance, Klebsiella pneumoniae represents one of the most concerning pathogens involved in
antibiotic resistance and as such, together with other highly important MDR pathogens. Recently, World
Health Organization also warned the community that multidrug resistant bacteria are emerging worldwide
which is a big challenge to healthcare.

Objectives of Study:

The key objectives of the study are:


 To check the prevalence Klebsiella pneumoniae among hospitalized patients
 To check the antibiotics suspectibility pattern of Klebsiella pneumonia strains against high
generation of antibiotics
 Molecular detection of Klebsiella pneumonia of drug resistance genes
Hypothesis:

Plan of Work:
Sample
Total of 100 clinical samples of pus, swab, urine, Blood, catheters, bronchial washings and CSF
etc will be collected from hospital and utilized.
Sample processing
Isolation of K. pneumonia isolates will be culture.
Biochemical Test
Antibiotics sensitivity test
Antibiotic sensitivity of clinical K. pneumoniae isolates will be done by Bauer’s and Kirby’s disc
diffusion method. Some of the antibiotics that will be used is mentioned in Table 1.

DNA Extraction
DNA extraction will be carried out using Thermo scientific DNA extraction kit as per
manufacturer’s guidelines.
POLYMERASE CHAIN REACTION
The primer will be selected and design according to antibiotics sensitivity pattern of Klebsiella
pneumonia strains.
