    GMFM

    Brief introduction:
    • Criterion-referenced observational measure
    • Not Norm-referenced、Age-reference
    • Mainly assese:腦性麻痺(CP)
    • normal children by the age of 5years can generally accomplish all of the
    items.

    • Purpose:
    1.Describing a child‘s current level of motor function.
    2.indicate the changes between pre.&post. test.
    3.Determining treatment goals.
    4.Providing easy explanations to parents.
    GMFM-88 V.S. GMFM-66
    GMFM-88
    • 適用 CP,Dawn syndrom, acquired brain injury
    • 較 66 更能反映兒童能力。
    • 臨床上用來評估同一名兒童某時間點的能力或經一段時間的變化。
    GMFM-66
    • 只能用在 CP
    • 較省時(項目較少)
    • 做研究統計使用
    • 可做不同兒童同一時間的比較
    Reliability and validity
    • GMFM-88
    於 CP 兒童之再測信度:0.75~0.97、測試者間信度:0.87~0.99
    • GMFM-66
    於 CP 兒童再測信度：0.99

    適用年齡與測試內容
    • 適用年齡、對象: ≦DA 5Y CP
    • 建議每半年評估一次
    5 dimension:
    1) lying and rolling(17 items)
    2) Sitting(20 items)
    3) Crawling and kneeling(14 items)
    4) Standing(13 items)
    5) Walking,running,jumping(24 items)
    • 項目的編制大致上依據發展順序，由簡單到困難
    Scoring
    The child is allowed a maximus of three attempts,choose the best performance.
    4 point scale:
    • 0. does not initiate(0%)
    • 1. initiate(<10%)
    • 2. partially completes(10%~100%)
    • 3. completes(100%)

    Precaution of Scoring
    • Any item that has been omitted or that the child is unable(or unwilling )to
    attempt must be assigned a “zero”
    • The child is allowed a MAX of three trials for each item.Any spountaneous
    performance is allowed but must be included as one of the three trials.
    • Any item cannot be score based on the scores achieved on subsequent items.

    GMFCS
    Reliability and validity
    • 測試者間信度，<2 歲:0.55，2~12 歲:0.75
    • 穩定度研究顯示 73%CP 兒童維持在同一層級，若一開始被評為階級 I 或
    V，較不會更動。
    • 6 歲以下的 CP 兒童，後來階級若有改變，通常會變得更差。
    How to determine GMFCS Level?
    站姿能力、坐姿能力、移位能力
    • 可自行坐在地面上跟椅子上
    • 上下椅子需協助
    • 有交替向前爬動作但可能非主要移位方式
    • 無法將自己完全拉到站姿
    • 無法自行行走
    • 判斷可能是 GMFCS Level3
