KEY STAGE 1 - 3RD QUARTER CONTEXTUALIZED ASSESSMENT

On the preparation and checking of Forms, DepEd Order No 11, s. 2018 Guidelines on
the Preparation and Checking of School Forms shall be strictly observed.
a.To facilitate timely, consistent, efficient and quality checking of school forms, the Division
will implement a per municipality approach in the division level checking of forms. The
composition of the Division Checking Committee (DCC) is as follows:
b.The Division Planning and Research unit will provide technical assistance on updating of
End of the School Year (EOSY) in the Learner Information System.
c. Public Schools District Supervisors shall serve as Vice Chairperson of Division Checking
Committee in their assigned districts. Likewise, they shall sign all forms in behalf of the
DCC Chair including private schools.
d.Moreover, District LIS Coordinators shall be the deputized members in the Division
Checking Committee in behalf of the Division Planning Officer.
e. The DCC shall conduct the checking activity for grade levels that shall move up/graduate
(Kinder, Grades 6, 10 & 12) prior to the school’s schedule for the moving up
ceremony/commencement exercises. The checking for other grade levels may start after the
4th quarter examination.
f. Division Checking Committee or its deputized member shall follow the size of the schools in
identifying the number of classes to checked and read. Please see the attached list of
schools with their corresponding school size.
END OF THE SCHOOL YEAR 2022-2023 ACTIVITIES
For the School Checking Committee (DCC):
a.The School Head as the Chair of the SCC has the discretion to reconstitute the members
of the committee, except for the LIS Coordinator as one of the two co-chairs.
b.The SCC shall be responsible for checking the school records, make necessary
adjustments, and affix their signatures the School Form 5 (Report on Promotion).
c. After checking the school forms, the SCC shall submit to the DCC copies of the following
forms on or before July 30, 2023. Due to the size of the Division, School Forms shall be
consolidated by the Public Schools District Supervisors thru the District LIS Coordinators
for submission to the Division.

END OF THE SCHOOL YEAR 2022-2023 ACTIVITIES
On 2022-2023 End of the School Year Rites which are scheduled on July 10-14, 2023,
the conduct of activities shall focus on the theme Gradweyt ng K to 12: Hinubog ng Matatag na
Edukasyon (K to 12 Graduates: Molded through a Resilient Educational Foundation) reiterating the
following policies:
a.Observance of physical distancing shall no longer be mandatory while wearing of face mask
during ceremony is not required and shall be left to the discretion of the learners and their
parents.
b.Graduation and Moving Up Ceremonies should be simple yet meaningful. While these rites
mark a milestone in the lives of the learners, these should be conducted without excessive
spending, extravagant attire, or extraordinary venue.
c. Non-academic projects articulated in DO No. 66, s. 2017, entitled Implementing Guidelines
on the Conduct of Off-Campus Activities such as attedance to field trips, film showing, JS
Promenade and other school events should not be imposed as requirements for graduation
or completion.
END OF THE SCHOOL YEAR 2022-2023 ACTIVITIES
d.For Public Schools, expenses relative to the activity shall be charged against School
MOOE/local Fund subject to existing accounting and auditing rules and regulations. No
DepEd personnel shall be allowed to collect any kind of contribution or fee for
graduation/moving up/completion ceremony.
e. Moving up or graduation rites shall be conducted in an appropriate solemn cermeony
befitting the graduating students and their family and shall not be used as a political
forum, in strict complaince with DO No. 48, s. 2018 entitled Prohibition of
Electioneering and Partisan Political Activity.
f. Public Schools District Supervisors shall submit in the SDO the schedule of graduation
of schools in their districts on or before June 23, 2023.

2. The aforementioned activity aims to determine the level of proficiency in the set of
competencies prescribed by the Department of Education (DepEd) for every subject/learning
area per grade level of DepEd Quezon learners. Likewise, the administration of 2023 Division
Achievement Test has the intention of gathering data that are useful for making administrative
decisions.
3.The expected examinees are DepEd Quezon learners from Grades 1 – 12 who will be composed
of 40 randomly chosen learners per grade level for big schools, 30 randomly chosen learners
per grade level for medium schools, and 25 randomly chosen learners for small schools. The
test administrators are SHs from other schools, within the district while the monitors are teams
of Education Program Supervisors and Public Schools District Supervisors designated by the
Schools Division Superintendent through the Chief of Curriculum Implementation Division.
