Original and Photo copy of 10th Board Certificate Hsc Exam or equivalent OR Original 10th Board Mark Sheet-cum-Provisional Certificate of HSC Examination or

equivalent as the case may be.
3 Original School Leaving Certificate (SLC)/ College Leaving Certificate (CLC) from the institution last attended.
4 Original Conduct Certificate from the institution last attended.
5 Three (3) recent colour passport size photographs.
Original and Photo copy of ST / SC Certificates from revenue officer not below rank of Tahasildar/Additional Tahasildar. For ST applicant, his/her father’s caste certificate

may be taken into consideration. Land passbook authenticated by Tahasildar/Additional Tahasildar may also be taken as a proof of caste in absence of caste certificate.
7 Original and Photo copy of Migration Certificate for the applicant from other Boards except BSE, Orissa.
8 If you want to go your higher preferred HSS then submit your request in student login on "Slide-up request" form after admission updated by HSS.

ଅନଲାଇ ମା ମେର “Slide-up Request” ଫମ ଦାଖଲ କରିବାକୁ ପଡ଼ିବ ।
ଆେବଦନକାରୀ ପ ୀକୃ ତ େମାବାଇ ନମରକୁ ପଠାଯାଇଥବା OTP ମା ମେର ହ ଏହି ପକିୟା
ପାମାଣିକରଣ େହବ।

If an applicant desires to avail Slide-up option after taking


admission in a lower choice of preferred Higher Secondary School,
then s/he have to submit the “Slide-up Request” form through on-line
in his/her registered login on SAMS Portal
(http://samsodisha.gov.in). The process will be authenticated
through OTP sent to the registered mobile number of the applicant.
