Bishop Score

The Bishop score is used to determine if a woman's cervix is favorable or unfavorable for labor and delivery. It assigns points based on dilation, effacement, consistency, position, and station of the cervix, with higher scores indicating a more favorable cervix. Modifiers are added or subtracted based on factors like preeclampsia, prior vaginal deliveries, postdates pregnancy, and premature rupture of membranes. A score under 5 indicates the need for cervical ripening, while a score over 5 is favorable for labor induction. A score over 8 means the chance of vaginal delivery after induction is similar to spontaneous labor.

BISHOP SCORE

Used to determine the status of the cervix-favorable/ripe or unfavorable/not ripe

POINTS 0 1 2 3
Dilation Closed 1-2 3-4 >4
Effacement 0-30% 40-50% 60-70% > 80%
Consistency Firm Medium Soft
Position Posterior Mid Anterior
Station -3 -2 -1,0 +1, +2

Modifiers

Add 1 point to total score for:

Preeclampsia
Each prior vaginal delivery

Subtract 1 point from total score for:

Postdates pregnancy
Nulliparity
Premature or prolonged rupture of membranes

Interpretation

A. Indications for Cervical Ripening with prostaglandins (NOT IF VBAC!)

1. Bishop Score < 5 (unfavorable/not ripe)
2. Membranes intact
3. No regular contractions
B. Indications for Labor Induction with pitocin
1. Bishop Score > 5 (favorable/ripe)
2. Rupture of membranes

If Bishop Score > 8, the probability of vaginal delivery after induction is similar
to that after spontaneous labor.
