    Jhon Ryan Lingahan, a field officer for RGS Recovery Management & Collection Services, Inc., provided an affidavit regarding a repossession request for a motorcycle owned by Joseph Herrera Quintana. According to the affidavit, Mr. Quintana had not paid the outstanding balance or monthly collections for his motorcycle since October and November 2023. The motorcycle was also involved in a traffic violation in August 2023 where the driver, Mr. Quintana, was not wearing a helmet and did not have a license or registration.     AFFIDAVIT OF UNDERTAKING
    I, JHON RYAN LINGAHAN, of legal age, Filipino with residential address at Brgy.
    Mayao Silangan, Lucena City, Quezon, after having been sworn in accordance with the law
    hereby depose and state that:

    1. I am the Field Officer 0f RGS Recovery Management & Collection Services, Inc. with its
    main branch office at Gilmore K Points Bldg., Corner Aurora Blvd., Mariana Quezon City ;

    2. That I am in custody and at the same time elucidating whether the vehicle owners are
    paying the monthly collection for their acquired vehicle units in the different parts of the
    Quezon Province;

    3. That one of our vehicle particularly MOTORCYCLE KAWASAKI WITHOUT SIDECAR


    bearing Plate No. 297OXJ is registered in the name of JOSEPH HERRERA QUINTANA
    and he was not able to pay the balance for the month of October,2023 with an estimated
    amount of Php42, 347.24 and the monthly collection amounting toPhp3,356.51 since
    November,2023;

    4. That above described motor vehicle driven by JOSEPH HERRERA QUINTANA was
    involved in a traffic violation that occurred on August 25, 2023 approximately 8:30 o’clock
    in the morning along the National Highway of Arias, Tayabas, Quezon whereby the
    aforesaid JOSEPH HERRERA QUINTANA was driving without helmet, with no Driver’s
    License and No OR/CR, wearing slippers and no Plate number of the abovesaid vehicle.
    Such fact is embodied in the office receipt hereto attached as Annex “A”;

    5. Because of such incident, the vehicle was impounded by the authorities in the Camp
    Guillermo Nakar, Brgy. Gulang-Gulang, Lucena City, Quezon;

    7. This affidavit is being executed to support the request for Repossession of the impounded
    vehicle unit with Plate No. 297OXJ;

    8. Further, I am executing this undertaking to attest to the truth of the foregoing and for
    whatever legal purpose it may serve.

    AFFIANT FURTHER SAYETH NAUGHT.

    IN WITNESS WHEREFOR, I have hereunto set my hands this 19 th day of December,
