    PHC AND CHC


    What is a PHC?
    “It is essential health care made universally accessible to
    individuals and acceptable to them, through their full
    participation and at a cost the community and country can
    afford.”

    Comprehensive Primary Health Care(CPHC)


    Ayushman Bharath Programme (AB) - initiated to upgrade
    all Health center's as Health and Wellness Centers
    (HWCs)
    Types of PHC’s
    Objectives
    1. High quality services that are accountable,
    responsive, and sensitive to the needs of the
    community.
    2. To specify the minimum assured (essential) and
    achievable (desirable) services.
    3. To provide guidance on health systems
    strengthening components
    4. To achieve and maintain an acceptable standard of
    the quality of care at public facilities
    5. To facilitate monitoring and supervision of the
    facilities
    6. To provide guidance and tools for governance,
    leadership and evaluation.

    Objectives

    Classification
    Non-FRU CHCs (rural):
    Essential services- preventive, promotive, curative,
    palliative, and rehabilitative services

    FRU CHCs (rural and urban):


    • Specialized care (physicians, surgeons, obstetricians,
    pediatricians, and anesthesiologists)
    • Accompanying infrastructure (functional operation
    theatre and blood storage unit)
    • Elective and emergency surgical services of secondary
    level care

    Population Norms For CHCs

    Rural areas (CHC)


    • Population norm of 80,000 (in hilly and tribal areas)
    • 1,20,000 (in plains) and/or time to care approach

    Urban areas (UCHC)


    • Should be established at the:
    Ward/Town/ULB/Block/City/District level to establish
    effective convergence and linkages with citizen centric
    services
    • Metro cities with a population of 5 lakh and above
    • Non-metro cities with a population of 2.5 lakh
    Services
    • Services should be offered & distributed across the
    district
    • Apart from curative services, there should a strong focus
    on health promotion, prevention, palliation, and
    rehabilitation at both primary and secondary levels of
    facilities
    • Non-FRU CHCs- services related to maternal and child
    health, infectious diseases, nutritional disorders including
    iron-deficiency anemia, mental health conditions, non-
    communicable diseases, Eye, Oral and ENT care
    • FRU CHCs include specialist care, operative services
    and blood transfusion facilities in addition to maternal and
    child health care services

    The HRH for CHCs is summarized under the following


    heads:
    • 1. Administrative staff
    • 2. Doctors (Medical Officers and Specialists)
    • 3. Nursing staff
    • 4. Allied and health professionals
    • 5. Support staff
    Human Resources
     Doctors (Specialists and Medical Officers)
    • Non-FRU CHC - 1 Essential (preferably General
    Medicine)
    • FRU CHC and UCHC- 5 Essential specialists(General
    Medicine, General surgery, OBG, pediatrician and
    anesthesiologist) for specialized care.

     Nurses
    • In Patient Services- 1 nurse for every 6 beds in the
    general wards (3 shifts=3)
    • Operation Theatre - 2 nurses per OT shift (3 shifts=6
    nurses)
    • Elective OT (1 shift= 2 nurses)
    • Nurse in-charge of the OT complex
    • Additional 2 nurses for the common pre- operative and
    post-operative areas
    Human Resources
    • 1 nurse/midwife for every two LDR beds. In addition, 1
    trained neonatal nurse every 4 neonates
    • In OPD clinics, every patient will go to the nursing station
    to get their vitals checked
    • Assist the doctors in- examining the patients (female
    patients)
    • Nurses will explain treatment and prescription to the
    patient

    Allied Health Professionals


    • The clinical psychologist and physiotherapists -
    (essential) FRU & Non-FRUs
    Human Resources

    • Social worker -(essential) FRU-CHCs in urban and rural


    areas
    • Dieticians- advice & support in IPD/OPD and will also
    need to provide dietary advice and guidance for NRCs
    • Counsellors - cover requirements of the major national
    health programmes
    • Medical Laboratory Technologist/Lab technician
    • Routine tests in the clinical/central lab, emergency unit,
    programme tests, etc.
    • Functions 24/7
    Human Resources
     Pharmacist
    • Dispense-120 prescriptions per day
    • Maintain the drugs and equipment inventory

    • One OT technologist/OT Technician and TSSU assistant
    per shift
