Possible Final Exam Questions

Administrative information systems support hospital operations. Workload measurement systems help managers plan budgets and determine staffing needs. The ADT system inputs patient information like demographics and clinical data at registration. CDI specialists submit queries to physicians but do not assign codes. Disparate systems located in different departments can cause accessibility issues. Integrating systems has advantages like reduced costs and improved care, but also disadvantages like transition costs and delays. Hospitals must follow privacy, security, and best practice standards.

RECM1001 - Post Intercession Week possible test questions

Administrative information systems support the operations of a hospital.

True

Managers of the departments within a hospital would use the workload

measurement systems when planning a budget for new employees.
True
Workload measurement = system where they can calculate how many sta is
needed, how many physicians are available for surgery, etc.
Workload = quantity of employees
Human Resources System = quality of employment (salary, contracts, etc)

ADT system is most often considered a disparate system

False
ADT - has to be up to date and share information, need to be able to send
information to other systems in real time
Demographic data is in the ADT system
ADT system = where the patient’s information is inputed at time of registration,
MRN #, complaint (clinical data)

Which of the following is not a function of CDI specialists?

A) submit queries to physicians requesting clari cation of the documentation
B) assign the ICD 10 CA and CCI codes
C) Ensure the accuracy of quality of care by improving the documentation
D) promote the health care funding and resources
E) all of the above
Answer: B

Disparated systems are problematic - true or false?

-true, they are problematic- stand-alone systems
so doctors need to access data from multiple systems - false?
*ambiguity of the question- will delete the question

Ethical question-
answer is “none of the above”
has nothing to do with what is right/wrong, how you feel about something, not
to do with religious beliefs (although yes sometimes)

Scenario about CHIMA ethics

SWOT - Strengths, Weakness, Opportunities, Threats

Strengths-internal=great budget, experienced sta ,

External-other hospitals have already implemented the same system

Weakness-internal=limit budget, new sta or not enough sta to handle

backlog,
External-a new product no one has tried yet

Opportunities - internal-AI tool for transcription saves $, faster turn over of

dictated reports, moving sta to work at home, saving $ on space in the HIM
management
External-having updated system may entice new employees

Threats-internal-privacy risks, cyber attacks, sta may leave, costs increase,

disruption of productvity

Weaknesses = re ection on the management team, not doing their job properly,
work ow will be a ected,
if the hospital did not recognize weaknesses such as a limit budget, then they
could not plan properly for the implementation. If they did not recognize they
needed new sta or not enough sta to handle backlog this would cause a
decrease in productivity, inaccuracy of data. They also need to recognize their
weakness in training is needed, equipment a new product no one has tried yet.

Integrating a Disparate Information systems-2 advantages and 2 disadvantages

Disparate system=a stand alone computer system, or “Legacy systems” which

are problematic because its one computer system that is not linked causing the
health care providers to physically move from 1 place/department to other to
access patient information to access the complete health record

Advantages of integrating - reduce cost of duplicating services, having

information in real time allowing doctors to see test results in order to prescribe
medication and to treat patient in real time; better quality of care

Disadvantages - cost $$ to switch to a new system, learning curve for sta ,

causing a delay in productivity, or error when learning the system, all health care
professionals need to be trained, productivity may be decreased during the
transition period, possible risk of losing data during the process or breach of
information during downtime or during the transitional period.

Q. True or False - the government does not have any tracking system on the
hospitals - False; the hospital tracks.

Computer assisted coding tool - the tools the coding specialists may be using
for coding
Software - ICD-10/CCI
Tool - computer assisted coding tool; the computer selects the codes

3 Main purposes of implementing technology in the HIM department

-managing the work ow
-coding/abstracting
-managing the electronic health records or managing the data

Best practices
-accuracy of the data, following the standards for the de nitions of the
elements, data integrity, process of auditing/editing of the machines/electronic
health records,
-standardization of what is collected, auditing process for accuracy

What are the 3 main purposes of implementing technology in the HIM

department?
1. facilitating coding and abstracting
2. managing work ow in dictation and transcripion
3. managing records - managing the data or information (data management)

Know security or privacy safeguards!

1. Administrative safeguards - policies and procedures, contracts, plans for
security, policies associated with information access including appropriate
clearance procedures, termination procedures, members of the workforce,
backup plans, disaster recovery plans
2. Physical safeguards
3. Technical safeguards

Best Practices - “procedures” or actions that are recommended, endorsed or

suggested as being the most e ective
• Data Integrity
• Provincial and federal laws and regulations
• A standardized format
• Data Dictionary
fl
fl
ff
fi
• Access permissions to the data

1. access permissions to the data referred to as create, read update and

delete (CRUD authority - clear policies on who can document and update
information in the EHR
2. having a data dictionary with standard data eld de nitions on the EHR for
each data element
3. have standardized format on the EHR to ensure consistency and accuracy
of the data
4. ensuring the organization follows the provincial and federal laws, rules and
regulations, and organizations policies, to ensure con dentiality and privacy
of the information on the EHR
5. ensuring data integrity policies and procedures are followed by people or
machines by having an auditing process on the EHR

Quiz Week 11
What type of safeguard would assist in preventing a coder from entering the
wrong ICD 10 CA code on the DAD?
-technical safeguard - edits and audits are technical safeguards required to be
on the abstract database to prevent errors

What is the process used to review, collect, analyze, and translate data for the
best possible decisions that can be made about the health of patients?
-Evidence Based Medicine

Give 3 di erent emerging technologies used in health care environments that

support automatic recognition without using computer keyboards
-bar coding
-optical readers
-voice recognition

Two bodies of law relevant to HIMs

-Administrative and Health Law
Administrative law - deals with the relationship between the government and
their citizens
-eg. Subsidized housing was denied to an individual because the individual
violated something, minimum wage, bene ts or lack of bene ts
Health law - impacts health care;
Eg. Negligence of a doctor caused by operating on the wrong leg; health
insurance fraud such as claiming illness when not sick

Two examples of administrative safeguards needed when implementing the

electronic health record
ff

-termination policies and procedures when sta do not abide by the law
-policies and procedures for breaches to ensure …
-procedures for all the steps when implementing the EHR
-back up plans and procedures when implementing the old system with the new
system

Incorrect diagnosis due to transcription error (AI)

-incompatible technology -> operational risk
-data integrity ~ security risk

True or False:
Delay in the implementation of an EHR is a security risk
False - is a project risk

True or False:
The incompatibility of the technology is a project risk
False - incompatibility is operational risk

True or false:
Code grey is a hospital code for breach of information
False - operational risk, since system failure/crash
If code grey is a result of a cyberattack -> also a security risk
