FCRA Authorization
You are receiving this Authorization because you have applied for employment with one of the following
operating companies in the FedEx enterprise:
FedEx Corporation
FedEx Services
FedEx Express
FedEx Freight
FedEx Trade Networks
Throughout this authorization, any reference to “the Company” means the specific FedEx operating
company listed above for which you submitted an application of employment.
AUTHORIZATION
1. Authorizing the Company to obtain and use background information on me from a consumer
reporting agency, in the form of consumer reports and/or investigative consumer reports, on a
continuing basis, solely for employment purposes either in connection with my job application
or with any future decisions concerning my employment with the Company;
2. Consenting to the release of consumer reports and/or investigative consumer reports to the
company for employment purposes either in connection with my job application or with any
future decisions concerning my employment with the Company;
3. Agreeing that this authorization remains in effect indefinitely until it has been revoked in
writing;
4. Certifying that I have received a copy of the FCRA Disclosure and have read and understood that
document;
5. Certifying that I have carefully read and understand this authorization and am executing this
authorization voluntarily and with the knowledge that the information obtained pursuant to this
authorization could affect my eligibility for employment; and
6. Certifying that my election to check the box and complete this acknowledgment electronically
constitutes an electronic signature establishing my consent to and execution of this
authorization.
CALIFORNIA: Under California law, the consumer reports we order on you for employment purposes in the
State of California are defined as investigative consumer reports. These reports may contain information on
your character, general reputation, personal characteristics, and mode of living. Section 1785.22 of the
California Civil code provides you the right to view and copy the file maintained on you. Because state law
provides for you to receive your report, the Company has arranged to have the Consumer Reporting Agency
automatically send you a free copy of your report. No action is required by you.
MAINE: Upon request to the Company, you have the right to know whether the Company ordered a
background check report on you. You may request the name, address, and telephone number of the nearest
FAIR CREDIT REPORTING ACT AUTHORIZATION
office for the Consumer Reporting Agency. The Company will send this information to you within five
business days of our receipt of your request. You have a right to ask the Consumer Reporting Agency for the
report.
MASSACHUSETTS: Upon written request, you have the right to a copy of any background check report
concerning you that the Company has ordered. You may contact the Consumer Reporting Agency for a copy.
MINNESOTA: Because state law provides for you to receive your report, the Company has arranged to have
the Consumer Reporting Agency automatically send you a free copy of your report. No action is required by
you.
NEW YORK: Upon written request, you have the right to be informed of whether or not the Company
requested a background check report on you from a Consumer Reporting Agency. If so, you may inspect and
receive a copy of the report from the Consumer Reporting Agency. Also, by executing this authorization, you
acknowledge receipt of Article 23-A of the New York Correction Law.
OKLAHOMA: Because state law provides for you to receive your report, the Company has arranged to have
the Consumer Reporting Agency automatically send you a free copy of your report. No action is required by
you.
WASHINGTON: Upon written request made within a reasonable time after this authorization, you have the
right to receive from the Company a complete and accurate disclosure of the nature and scope of any
investigative consumer report the Company may have requested. You also have the right to request from the
Consumer Reporting Agency a written summary of your rights and remedies under the Washington Fair
Credit Reporting Act.
