RULES AND CONDITIONS


1. Outstanding Balances Must be fully settled first at the Property Management Office (If there’s any)
2. Required Documents Must be complete and valid
3. Property Insurance Comprehensive Personal Liability or Third-Party Liability Insurance
4. Fire Extinguisher 10lbs ABC type available in the unit/house, quantity based on fire code requirement
5. Welcome Home Orientation Must be done prior to move-in or a maximum of seven (7) days after move-in
6. Pet Policy Pets are not allowed as stated in 2.5 Section A of the Master Deed and Restrictions
RESIDENT’S AUTHORIZATION TO ACCESS THE UNIT DURING EMERGENCIES
I hereby authorize The Residences at Greenbelt Condominium Corporation (“TRAGCC”), through the Property Management Office, or its authorized personnel, to enter my
unit/property, by any means necessary, to prevent the spread of damage to the common areas or to other units during an emergency such as fire, flooding, and other life-
threatening situations. For non-emergency cases, such as in the event of an incident or suspected incident inside my unit/property that could compromise the life, safety,
and/or health of the other residents, I hereby authorize the Corporation, upon written notice to my last known mailing address or e-mail, to enter my property, by any means
reasonably necessary to enjoin and abate such threat to life, safety or health. I hereby agree to hold the Corporation, its directors, and/or officers, free and harmless from
any liability or claim which may arise, or for any damage that I may sustain, as a result of such authorized and justified entry.

CERTIFICATION AND DATA PRIVACY CONSENT


I hereby certify that I am the same person named and identified in this form and that the data/information I have provided herein is true and correct. I hereby authorize and
give my full consent to The Residences at Greenbelt Condominium Corporation (“TRAGCC”) and its authorized contractors or service providers, if any, to collect, store, process,
and retain the personal data and information provided herein for any of the lawful purposes authorized by Corporation’s Articles of Incorporation, By-Laws, Master Deed with
Declaration of Restrictions, House Rules and such other rules and regulations which may now or hereinafter be adopted by the Corporation. I understand and acknowledge that
the collection, storage, and disclosure of my personal data and other personal information is necessary for the Corporation to grant me access to the Condominium and any of
its facilities. Further, I hereby warrant the correctness, completeness, and accuracy of the personal information provided herein, and that I have secured the consent of those
individuals whose personal information appears in this form. I shall hold the Corporation, its directors, and/or officers, free and harmless from any and all liability arising from
my failure to provide such correct, complete, and/or accurate personal information. I am aware of my rights as a Data Subject under the Data Privacy Act of 2012, particularly
Section 16 thereof, including my right to access, update, make corrections to, rectify, erase, or block my personal information. I understand that I have the right to file a
complaint before the National Privacy Commission when my rights as a Data Subject have been violated. Should I have any concerns about the collection and processing of my
data and personal information, I shall contact the Data Protection Officer of the Corporation at the contact details provided in the Corporation’s Data Privacy Policy Notice. I
understand that my consent does not preclude the existence of other criteria for lawful processing of personal data, and does not waive any of my rights under the Data Privacy
Act of 2012 and other applicable laws.
