**Subjective – what the patient stated, a format will be given for the Review of Systems (ROS),
just edit, do not delete
**Objective – what the doctor have observed/checked/stated about the patient. There is also a
template for each ROS, you can delete those parts that the doctor did not check.
**Assessment – Diagnosis of the patient
**Plan – Treatment, state lab procedures that the doctor ordered. For example, CBC, MRI, UA, FA;
verb should be past tense
