When more than 50 persons are Employed, First Aid facilities run by a male nurse familiar with first

aid and
cardiopulmonary resuscitation (CPR) requirements and exclusively assigned to medical duties.
Will provide a dedicated emergency vehicle (Ambulance), properly supplied and marked, to transport injured or
sick personnel to the nearest designated Hospital or clinic. The Ambulance will be equipped with the following
supplies.

 Stretcher and Blankets.


 Portable Oxygen.
 Splints for Bone fractures.
 Bandages / rubber Tourniquet.
 Sterilize Wash Water.

Will post notices indicating the following:

 Name of the person who is in charge of the first Aid cabinet.


 The hospital to which injured person that requires hospital treatment is to be sent.
 The telephone number of the Doctor and male nurse.
 The emergency telephone number to be called for Assistance.

10. Incident & Investigation Program


Purpose:

The purpose of this Procedure is to establish a consistent means of recording accident/incident investigation
information, which will be used to prevent a recurrence of the same or similar accidents.
The CRCC is committed to the protection of its employees, the environment and its physical assets. The CRCC will
continue to maintain a safe work environment in order to prevent occupational injuries and illnesses.

Scope:

This Procedure applies to all CRCC Locations & its employees, and is inclusive to all injuries/accidents that occur in
the course of work, and/or within the workplace.

Policy:

It is the policy of the CRCC to thoroughly and immediately investigate all injuries/incidents and report all required
information to the Workplace Safety and Insurance Board and Client (CLIENT’S), if applicable including:

 Fatalities
 Critical Injuries
 Lost Time
 Medical Aid
 Occupational Illness
 Property Damage
 Fire
 Environmental Release
 Workplace Violence and Harassment

Responsibilities:

Project Manager will

 Assure immediate medical assistance where required; and,

 Report the injury or incident to the Health & Safety Coordinator and Administrative Area Manager
immediately so that the investigation can be conducted and actions can be taken to prevent future similar
accidents/injuries.

 He will participate in an investigation of an incident if the severity or potential severity requires action
appropriate to the manager's authority.

 All investigations requiring immediate notification to Client will be attended by the Project Manager. Copies
of investigations required by the Client will be provided by the Project Manager.

Supervisor:

 A supervisor must advise new and returning workers of the requirement to report all incidents including
close calls.

 An annual reminder to all employees to report incidents is recommended. A supervisor must investigate
incidents in a manner that is timely and appropriate to the circumstances and severity of the incident. A
supervisor's incident review and signoff are a requirement.

Safety Supervisor

 Immediately investigate all accidents and incidents to the extent needed to determine cause and actions
needed to prevent a recurrence and to complete all sections of the Accident Investigation Report. Such
investigations are to include assessing the scene of accidents and interviewing of witnesses;
 Assist when occupational health and safety expertise is needed in completing corrective actions identified
by the supervisor;
 Ensure that training is provided in accident/incident investigation and reporting, as needed.
 Establish corporate guidelines for accident/incident investigation and reporting.
 Establish corporate supervisory training standards and monitor implementation.
 Review accident/incident trends to assist in identification and initiation of appropriate prevention efforts in
order to prevent a reoccurrence.
 Document the recommendations for improvements and corrections as generated by the Company
management as well as date of implementation.
 Communicate any changes in the system as a result of improvements following an injury/incident
investigation.
 Make recommendations based on root causes of injury/incident investigation findings in order to make
improvements and corrections in order to prevent any further incidents from occurring.

 Support the implementation of this policy by discussing injury trends, where known, with Human
Resources; and
 Protect the privacy of employees with respect to injury/illness/accident/incident information in their
possession.

Employee

 An Employee will report to the supervisor all incidents including Near Misses.
 An Employee will attend the incident investigation unless unable to do so as a result of injury.
 Workers may choose to report a Near Miss using the company's form Near Miss Reporting, or verbally to
their supervisor, who will be responsible for completing the document.

Incident Reporting and Investigations

CRCC - Co. has an obligation to make certain that all our employees, staff, contractors, volunteers and visitors are
aware of the importance of and requirements for reporting and investigating close calls, near misses and other
incidents. Investigations of close calls and near misses provide a learning and improvement opportunity to help
prevent someone from being injured. An investigation is held to prevent recurrence, not to place blame.
Incident Reporting and Investigation Procedure:

 The incident is reported by a person to the company. (An incident involving an employee may be reported
by others to the company.) The incident may be reported verbally or in writing.

 The incident site must be visited if possible and the site preserved until the investigation is complete, if safe
to do so. Photographs, sketches and other evidence collection should be undertaken promptly. Note:
Incidents requiring immediate notification to Client (CLIENT’S, etc.) will require communication with them
with regard to site access and preservation of evidence.

 The direct supervisor of the employee involved or the person who reported the incident will organize and
lead the investigation. The investigation must be carried out by those knowledgeable about the type of
work involved and, if reasonably available, with the participation of a Safety Officer / Engineer.

 The investigation team will include those appropriate to the severity or potential severity and type of
incident. (Refer to the Risk Assessment matrix on the Incident Investigation Form).The team may include
people not under the scope of this policy (for example a prime contractor representative if the incident
occurred on a worksite).

 The investigation will follow the Incident Investigation Template format, which includes root cause analysis.

 The investigation must be held in a timely manner. A preliminary investigation may be necessary if required
attendees are not able to attend due to injury or other reasons.

 Action required as the result of an investigation will be recorded using a Corrective Action Log (CAL) format
and tracked for completion by the Safety Department.
 Safety Department will evaluate any future risks that recommendations or corrective actions could create.

 Safety Department will monitor the effectiveness of any changes or implementations.
 Safety Department will communicate the recommendations and corrective actions to all relevant parties.
 All investigation reports will be forwarded to the Safety Department. The Safety Department will review the
reports for completeness and determine if additional investigation or distribution is required.
 An industry safety alert will be issued through the company if findings from the investigation could help
others prevent injury.

Near Miss Reporting

Scope

This procedure applies to all CRCC & its subcontractor staff and it specifies:

2. Individual responsibilities

Purpose:
This procedure provides a process for reporting, investigation, analyzing, and communication near misses in
client’s Facilities / operations. The objective is to use near miss information to identify corrective actions and
preventive measures to reduce injuries and losses.

Responsibilities:

 Safety Manager

 Ensure all aspects of the near miss program are complete, are maintained up to date, and are
implemented adequately.

 Encourage and support open reporting where individuals do not fear any negative consequences from
reporting a near miss.

 Ensure that corrective actions are properly implemented.

 Ensure quarterly (at a minimum) analyses and summaries of near misses are prepared and
communicate.

 Annually perform an internal review of the near miss program and make improvements as necessary.

 Safety Supervisors

 Promote immediate reporting of all near misses, ensuring employees and subcontractors that these
areas considered positive efforts.

 Ensure all near misses are documented and forwarded to the department head using the attached
near miss report form.

 Verify all action items resulting from investigations are implemented, and other necessary follow-up
action is completed.

 Safety Officer

 Provide technical information and assist departments in implementing an effective Near Miss
Reporting program in their workplace.

 Ensure all employees and sub-contractor (If any) understands the intent of this procedure and has
been provided appropriate orientation on the organization's near miss program.

 Upon being notified of a near miss. Determine if additional investigation is required and arrange for
the instigation to be completed.

 Ensure action items resulting from investigations are communicated to responsible parties and are
tracked to completion.

 Workers

 Report any item that might be considered a near miss to their immediate supervisor.

 Participate in the investigation and resolution of near misses as directed by their supervision.

General Guidelines:

 Near misses should be openly reported, investigated, analyzed, and communicated with the objective
of sharing lessons learned and taking corrective action to prevent recurrence and improve safety
performance

 All CRCC Company's Departments should promote and publicize that near miss reporting is a positive
attribute to the encouraged and supported. Employees must be made aware that they will not be
punished or disciplined for reporting near misses.

Reporting Procedure:

 Any CRCC employee or sub-contractor who believes a near miss has occurred should report the
incident to his supervisor as soon as possible. For contractors or maintenance personnel the report
should be made to the responsible client’s.

 Supervisor / foreman in charge of the facility where the incident occurred. The initial report may be
verbal, in writing, or by e-mail.

 CRCC employee and subcontractor should fill in a Near Miss Report Form Attached with assistance
from the supervisor if required. Should review and sign the form and ensure it is routed to the division
head.
