    Dear Ma’am/Sir,

    Good day!

    Barangay Health Workers (BHW) are frontline health workers who provide basic health
    education and selected primary health care services. BHWs are to fulfill the roles of advocate
    of health programs; educator to guide the community on health priorities of the city and
    provide a variety of contraceptive methods; disseminator of health updates; “coordinator”
    of health services; “record-keeper” of health data, activities and events in the community.

    And to help other barangay workers and the local officials, the BNS keeps a record of the
    results of the regular nutritional assessment as well as records on the nutrition and health
    profile of families in the barangay. The BNS also keeps records of the various nutrition
    programs and accomplishments of the barangay.

    In order to provide them with all the necessities they needed in performing their task.
    However, the barangay focal situation could not hold to fill that considering our budget.
    We wish to give them new set of uniform for the workers.

    In line with this, I would like to request from your benevolent heart for some donation in
    monetary form which will be used for the purchase of new set of uniform of our BHWs
    and BNS.

    I hope that you take this opportunity to support this cause.
