EXIT INTERVIEW QUESTIONNAIRE

NAME : Anthony Lim Flores Jr.

POSITION : Accounting Assistant
DEPT/SECT : Finance/General Accounting
DATE HIRED : July 06, 2023
LENGTH OF SERVICE : 2 months & 26 days
DATE OF SEPARATION : September 30, 2023

Directions: Please answer the following questions as honest as you can. This will not affect your
employment record with us, thus it will help us in improving our services/operations to other
employees. Rest assured that all your answers will be treated in utmost confidentiality. Thank
You.

1. What is the reason of your separation from the company?

We are not compatible as a team because we have different views and perspective on
how to do things but same objective to expedite the process to avoid building up of backlogs.

2. Did you have a worthwhile stay with the company? Why?

Yes, I have. I learned a lot from company industry and how it operates and I meet
people.

3. What is the common problem you had encountered during your employment with the
company?

Well, it’s very hard to start because of backlogs and that I need to develop first the
working paper rather than getting from previous work, for me to expedite the process. Its very
hard to rely also to somebody else since we have a lot of misunderstandings and the direction is
vague, resulting to waste of effort and a sudden loss of motivation. Also, a lot of work is
dependable on information given for the accomplishment of the whole thing, that’s why they
should convey that information very clear at the very beginning to avoid delays and a return of
question.

4. How is it being resolved?

This should be resolved by someone who have control of all accounting, from the overall
design, automation of the systems, processes and work functions, and the level of dependency
of work should be studied first at the very top, and as of now things aren’t clear at the very
bottom since I do understand that backlogs exist and need to consider and focus and its time
consuming.
5. Do you perform functions as stated in your Job Description? If no, why?

I do, but for some tasks it wasn’t given yet because there are so many things to consider
and it’s not that a priority, like the Bank Reconciliation, in short it will not be form part of my
regular task since it needs to develop first.

6. Please comment on the following:

6.1 Your Boss (immediate superior):

She’s kind personally, smart and good on presentation. She always finds sense in all she
does. The bad thing is when she tries to cover up or sided people in a negative way, by rant and
threat and humiliation, rather than alleviate the misunderstanding. She’s also not listening as if
all people aren’t getting what she tries to convey, and this result to waste of effort sometimes.
We end up on good terms and I wish her to fix accounting, and I understand her because of
backlogs and the set up is not there when she entered. I end my last day with no grudge.

6.2 Your Subordinate/s:

I don’t have.

6.3 Your workload:

It’s Fine.

6.4 Company Benefits:

Sufficient.

6.5 Salary Policies:

Enough.

7. Please rate the company according to the given factors. Your answer may be based on your
personal experience or observations from other department/s or company as a whole. Please
use the following rating scale:

2 C. Development of Cooperation and Teamwork
3 D. Resolving complains and problems
2 E. Encouraging open communication within the department/section
2 F. Development of employee
4 G. Provision for adequate employee benefits
8. What do you think are the changes to be done in order for the company to improve its
current operations and conditions?

I don’t have much to say about this, it depends on the leadership.
