Note: This Authorization is issued in accordance with DGCA CAR 145.A.35 (b) and AIESL, MOE section 3.4 and is subject to the following conditions.
The holder continues to be contracted to or in service with AIESL.
The holder is responsible to inform the office the Dy. QM whenever there is any change to any data reflected in this certificate.
The holder is medically fit to exercise the privileges of this authorization.
The authorization unless withdrawn/ cancelled, is valid for a period specified below, provided the holder’s License/ Approval and
continuation(Refresher) training remains valid during this period.
Certifying staff shall produce their authorization certificate to any authorized person within 24 hrs when demanded.
The authorization can be withdrawn at any time if deemed necessary by QM/ Dy. QM without assigning any reason.
This Authorization supersedes any previous authorization issued to the above holder.
The holder has read and understood the annexure in the reverse side of this authorisation certificate.
