Likelihood Table

This document contains tables to assess the likelihood, severity, and risk level of hazards. The likelihood table rates the chance of a hazard occurring from 1 to 4. The severity table rates the impact of a hazard from 1 to 4, from negligible to fatal. The risk matrix table combines likelihood and severity to determine an overall risk level of 1 to 16. Very likely Could happen frequently (Has a good chance of occurring and is not unusual) 4

Likely Could happen occasionally (Might be occurring sometime in future) 3

Unlikely Could happen but rarely 2

Highly unlikely Could happen, but probably never will 1

Severity Table

Severity Description Rating


Fatal Approximately one single fatality or major property damage if hazard is realized 4

Major injury Normally reversible injury or damage to health. (Non-fatal injury, permanent 3
disability)
Minor injury Normally reversible injury or damage to health requiring days off work. (Disabling 2
but not permanent injury)
Negligible Minor abrasions, bruises, cuts (first aid type of injury) 1

Risk Description Action


7-16 High A HIGH risk requires immediate action to control the hazard as detailed in the hierarchy of
control. Actions taken must be documented on the risk assessment form including date for
completion.
3-6 Medium A MEDIUM risk requires a planned approach to controlling the hazard and applies
temporary measure if required. Actions taken must be documented on the risk assessment
form including date for completion.
1-2 Low A risk identified as LOW may be considered as acceptable and further reduction may not
be necessary. However, if the risk can be resolved quickly and efficiently, control measures
should be implemented and recorded.
