1) This document is an instructional supervisory plan form used by the Department of Education in the Philippines to plan monitoring and evaluation of instructional supervision. 2) It includes sections to identify the instructional supervisor, school, objectives, target recipients, timeframe, planned supervisory activities and strategies, funding sources, expected outputs and outcomes, and spaces for signatures approving the plan. 3) The form is intended to be completed at the beginning of an academic year or quarter to outline the focus, monitoring activities, and expected results of instructional supervision for that time period.
